ACT Nurse Practitioner

Job Description

As a member of the Assertive Community Treatment (ACT) clinical team, functions as the psychiatric medical provider and senior clinician on the team, directing, coordinating, and providing medical/clinical services to members served by the team. Provides medication services, supervises teams delivery of direct clinical services, monitors member status, and participates in meetings, staffings, and treatment planning. Has a defined caseload, productivity goals, and meets the psychiatric needs of any member assigned to the ACT team. Provides after-hours care, on-call coverage, and support to all members served by the ACT team. Participates in utilization management and quality improvement activities as required.
All members of our team are expected to provide support and services in accordance with the Nine Guiding Principles for Recovery Oriented Adult Behavioral Health Services and Systems.

ESSENTIAL FUNCTIONS FOR THIS POSITION
? Serves as the attending medical provider for assigned members.
? Completes initial psychiatric assessments, supportive psychotherapy, engagement, medication management, progress notes, and annual psychiatric updates; conducts family meetings, interventions, and patient education; develops differential diagnostic assessments, assessments of capacity for consent to treatment, and risk assessments; monitors progress towards treatment goals including medical, psychological, social, behavioral, developmental aspects of the members situation; and assists with discharge planning.
? Assesses members needs and guides the clinical team while managing treatment plan updates, in addition to initiating contact with outside providers according to established guidelines.
? Orders follow-up blood levels and supportive laboratory tests according to standard medical practice.
? Reviews the health assessments completed by nursing staff and obtains past and current medical histories.
? Initiates contact with primary care physicians, emergency room clinicians and psychotherapists according to established guidelines.
? Utilizes appropriate medication based on best practices, in addition to appropriately completing required documentation for preauthorization of pharmacy services.
? Provides necessary information to clinical team regarding inpatient utilization.
? Works closely with other team members to ensure that each treatment modality is appropriate for the member in addition to working with outside psychiatric providers in crisis and inpatient services.
? Performs specialized duties as required by special clinical situations or treatment populations consistent with license and scope of practice, such as reviewing and recommending for MD sign off all amendments and PAD petitions originated at the clinic; evaluates members for continuation of court ordered treatment; prepares medical reports to the Psychiatric Security Review Board or Superior Court including treatment plan changes for those members on conditional release from Arizona State Hospital and receiving services at the clinic; and completes specialized treatment plans (STP) and Forced Medication Sheet (B form) whenever a member is placed on court ordered treatment or needs to return to inpatient treatment on an amended court order due to non-compliance.
? Actively participates in daily and individual team meetings and provides leadership in discussion and problem solving of members complex clinical or medical issues.
? Works collaboratively with the ACT clinical team to engage, educate, communicate, and coordinate care with each member, their family, behavioral, medical, and dental health care providers, community resource providers, and others to ensure that all services prescribed in the individualized service plan (ISP) are implemented.
? Works with members to develop roles for themselves outside of their mental illness, while focusing on expected outcomes of increased employment, meaningful community activity, independent living status, and social network, as well as decreased substance use/abuse.
? Actively participates in the clinical peer supervision process consisting of weekly one-hour supervision with the psychiatrist on difficult cases or as designated by the Chief Medical Officer.
? Pursues best practice outcomes for all members served.
? Works in collaboration with team members to assure compliance with SAMHSAs ACT fidelity measures.
? Practices according to evidence-based practices and AHCCCS Performance Improvement Protocols and Technical Assistance Documents as posted on the AHCCCS website.
? Documents each members progress in the clinical record, maintains each members comprehensive medical record, and accurately and thoroughly documents all services and activities in the medical record according to policy using approved forms and electronic systems.
? Conducts frequent community visits, including but not limited to private homes, Southwest Network sites, provider/treatment/service locations, educational facilities, jail/juvenile facilities, hospitals, and residential treatment/group homes.
NONESSENTIAL FUNCTIONS
? Practices according to the recovery model and knows and practices engagement skills including motivational interviewing and assists members to define and achieve their own recovery goals.
? Provides coverage to any other clinic as needed to meet business/clinical and member needs.
? Completes medication education and informed consent when possible.
? Actively participates in the clinical peer supervision process consisting of weekly one-hour supervision with the chief psychiatrist on difficult cases.
? Identifies and reports potential barriers to quality care and opportunities for improvement
? Participates in quality improvement activities as required including, but not limited to polypharmacy.
? Participates in medical staff meetings including regional and quarterly meetings, required site meetings, and any meeting as requested by the chief psychiatrist or chief medical officer.
? Follows policies and procedures and adheres to the requirements of the Corporate Compliance Program.
? Ensures confidentiality of verbal and written information in accordance with HIPAA standards and Southwest Network policy, and adheres to the legal, ethical, and professional guidelines adopted by Southwest Network.
