ACT Rehabilitation Specialist

The ACT Rehabilitation Specialist plays an important role on the Assertive Community Treatement team by helping the members they serve obtain and maintain involvement in employment and/or community-based activities. Youll provide vocational rehabilitation services, situational assessments, and support services to our most vulnerable members and serve as the subject matter expert for the team regarding rehabilitation- and vocational-related services. You will develop relationships with community employers, providers, or other organizations to foster an increase in employment and meaningful community activity opportunities for members.
The ACT model is a service delivery model, not a case management program. You will provide direct services to members and develop plans to help them reach their goals, and review each members progress daily. You will work with the ACT clinical team to engage, educate, communicate, and coordinate care with each member, their family, behavioral, medical and dental health care providers, community resources, and others to ensure that all services prescribed in each members individualized service plan are implemented. You must be passionate about pursuing best practice outcomes for members in all areas of their lives, including continuing education, employment, community tenure, social network, independent living status, and decreased substance use.

REQUIREMENTS
To be eligible for this position, you must possess a bachelors degree and one year full-time related work experience, or an associate degree and at least two years related work experience. Certified Psychiatric Rehabilitation Practitioner (CPRP) preferred. A minimum of one year experience working with job development and/or employment services and resources and working with seriously mentally ill adults is required. Prior Assertive Community Treatment (ACT) or equivalent experience is preferred.
Thorough and working knowledge of psychiatric and co-occurring disorders is imperative. Knowledge of DSM-5 codes and descriptions, psychotropic medications, and crisis intervention required. Knowledge of community based behavioral health services preferred.
Must be able to maintain excellent member and family relationships and provide excellent customer service. Requires effective interpersonal, written and oral communication skills. The use of sound judgment and excellent time management skills are essential. Must be able to follow direction, identify priorities and stay organized. Must be competent in the use of electronic medical records software, as well as in the use of a computer, printer, copier, and telephone. Preferred typing skills of 25 to 40 WPM. Must be highly proficient in the use of Microsoft Office software, including Outlook, Word and Excel. Must be able to effectively use the internet and various web browsers.
Must maintain current CPR, First Aid, and Therapeutic Options certifications, as well as self administration of medication certification. Must have and maintain a valid Arizona drivers license.
