SUMMARY

Under general supervision, provides direct and indirect, covered services and treatment to individual clients, coordinates with community supports and stakeholders. Responsible for managing appointment schedule, assigned caseload as a clinician, conducting assessments, implementing treatment plans, facilitating out-of-office services, performing crisis services when needed, and participate in ongoing learning experiences and treatment team consultation and coordination.

SPECIALIZATIONS

  • Working knowledge of lifespan development and harm reduction models.
  • Demonstrated ability to recognize symptoms to the full and broad range of DSM diagnoses.
  • Ability to provide routine, urgent, and emergent interventions.
  • Ability to apply and integrate the American Society of Addiction Medicine criteria in service planning and implementation.
  • Demonstrated ability to apply motivational enhancement, stages of change, family systems, and appropriate family communication process.
  • Ability to understand and screen for serious mental illness, and complete requests for determination.
  • Knowledge and ability to effectively engage clients in process and maintain safety and cohesiveness.
  • Knowledge and understanding of group therapy models and stages.
  • Ability to demonstrate education and verbalization of information in a culturally competent and sensitive manner.

PRIMARY DUTIES AND ESSENTIAL FUNCTIONS

Participate in a multi-disciplinary team in providing treatment for clients.

  1. To communicate with and coordinate services with other treatment providers when appropriate.
  2. To attend and actively participate in team meetings.


Manage and monitor individually assigned caseloads.
  1. Maintains personal contact with client/families at least every 30 days.
  2. Accurately documents progress notes the same day as service is provided, not to exceed 24 hours; crisis documentation completed same day as service.
  3. Document significant information in each client file in a clear and timely manner.
  4. Follow-up to service commitments, engagement, re-engagement for missed appointments, letters of intent, welfare checks, and appropriate discharge functions completed within required time periods.
  5. Maintains and monitors caseload statistical information, including presenting issues, length of stay, and outcome measures when assigned.
  6. Performs comprehensive intake and ongoing assessments.
  7. Accurately documents the assessment service, including assessment tools and progress notation.
  8. Conducts ongoing assessment, documents functional and symptomatic improvements.

Assist in working with clients with medication and case management services.
  1. Consults and coordinates with prescriber, laboratories, medical providers, and pharmacies, and assists with eligibility and access to prescription services when needed.
  2. Consults on initial and ongoing psychiatric evaluation requests.


Provide individual/family counseling and are able to facilitate groups. Engage clients, their families, and their community supports in treatment, provide illness self-management education, and coordinate services with referral sources and discharge planning.

  1. Coordinates and conducts group functions and facilitates cohesiveness in its entirety that demonstrate improvement in a clients/familys symptoms and functioning level.
  2. Demonstrates ability to de-escalate and de-brief disruptions to group and/or family content and process.


Facilitate and coordinate group therapy process.

  1. Able to promote individual/family culture.
  2. Utilizes informed consent in interventions, understands and is able to apply the value and limitation of didactic approaches.
  3. Collaborates, coordinates and consults with other involved service providers and treatment team members.

MINIMUM QUALIFICATIONS AND COMPETENCIES

  • Masters degree in a behavioral health related field
  • Two (2) years of full-time behavioral health work experience.
  • Possess a valid drivers license and ability to be insured under automobile insurance policy.
  • Ability to obtain fingerprint clearance card, CPR and First Aid.
  • Bilingual language skills in English/Spanish preferred, and as appropriate to population served.
  • Current license preferred, or ability to become licensed.
  • Must be 21 years of age.
  • Must successfully complete the agencys orientation and demonstrate competence regarding skills and knowledge.
  • Must maintain a professional appearance (business casual attire) and demonstrate appropriate time-management skills to include being punctual.
  • Must possess excellent phone etiquette, verbal and written communication skills.
  • Must be customer service driven and demonstrate the ability to multi-task.
  • Able to work with minimal supervision.
  • Ability to effectively utilize MS Office (Word, Excel, Outlook, Access).

Effective January 3, 2022, all CPLC Employees and all Service Providers, are required to be Fully Vaccinated against COVID-19. Newly hired employees, who are not Fully Vaccinated at the time of hire, are expected to begin a vaccination series immediately upon hire. CPLC recognizes medical and religious exemptions to the COVID-19 vaccination requirement, in accordance with applicable law and upon approval by CPLC. However, any person approved for an exemption, without exception, is subject to masking, weekly COVID-19 testing at their expense, and other requirements established by CPLC from time to time.

Competencies

  • Knowledge and understanding in general mental health, substance abuse, co-occurring disorders, and serious mental illness.
  • If working with youth, familiarity with application of behavioral principles in youth treatment, understanding of the Arizona Child and Family Team model.
  • Able to work a flexible schedule as needed per program requirements.
  • Have basic typing/computer skills.
  • Have excellent verbal and written communication skills.
  • Have good organization and time management skills.
  • If working with youth, ability to attend required Child and Family Team training within one month of assuming position and participate in ongoing learning requirements, as needed.

PHYSICAL DEMANDS AND TYPICAL WORKING CONDITIONS

Office Position: This position is both a mobile and a sedentary position with periods of light physical activity, and is performed mostly in office surroundings. Positions typical in nature require regular walking or standing; sitting, lifting and carrying up to 20 pounds; climbing stairs, bending, reaching, holding, grasping and turning objects; and using fingers to operate computer keyboards. This position will require the ability to speak normally and to use normal or aided vision and hearing.

Travel: Local, interstate and intrastate travel to Chicanos Por La Causa, Inc., its affiliates and/or subsidiaries, will be necessary to successfully fulfill the duties of this position. Candidate must have and maintain a valid drivers license and auto insurance. Technology will be utilized to minimize travel whenever feasible. The travel requirements for this position are anticipated to be 25 to 35 percent assignment depending. Overnight travel must be approved in advance by the Department VP.

Continuous Learning: All certifications related to the position at hire, or obtained while employed by CPLC, must be maintained. Candidate will be expected to participate in continuous learning opportunities to maintain competency and enhance skillset. Learning opportunities must be approved by Department VP.
