What You Bring/Job Requirements

Job Summary

Responsible for coordinating referrals and patient visits to the Valve Clinic. Utilizes discretion and independent judgment to facilitate and establish seamless care for patients who are candidates for trans catheter valve therapy from time of referral through pre and post procedure evaluation and follow-up. Applies the advanced clinical process to manage this subset of patients and coordinates numerous aspects of the Valve Clinic program while facilitating a team approach and collaborating with the multidisciplinary Valve Clinic team. This position requires marketing and communications with off-site local and outof town facilities to promote and coordinate the Valve Clinic Program. In addition, this position provides for optimal secondary preventative cardiac interventions. Will also work with Cardiothoracic Surgical office on an as needed basis for both the follow-up outpatient clinic and aortic surveillance clinic. Clinical coordination of other programs which requiresmultidisciplinary physician coordination and treatment prior to the procedure may be assigned. Responsible for reporting program outcomes on a regular basis to appropriate team members as well as initiating program improvement process development based on those outcomes. Demonstrates personal accountability for relationship-based care, theorganizational mission, and supports all core values, in addition to contributing to the program's strategic planning.

In addition to bringing your whole self to the workplace each day, qualified candidates will need the following

  • Two years Advanced Practice Provider experience required
  • Two years of work related to Cardiology/Heart Failure/Structural Heart experience preferred
  • Two years Marketing and/or Business Development experience, preferred
  • Communication and problem solving skills
  • Ability to communicate and work closely with Physicians

For PAs:

  • Masters graduate of an accredited Physician Assistant program required
  • PA Certification by N.C.C.P.A. and current mid-level provider license by the state of Colorado required
  • BLS from the American Heart Association required
  • ACLS from the American Heart Association may be required depending on practice setting

For NPs:

  • Master's Degree in Nursing as Nurse Practitioner, from a nationally accredited postgraduate certificate program required. Must be a graduate from an Accredited School of Nursing.
  • Graduate from an Accredited School Of Nursing required
  • Registered Nurse Practitioner Licensure for; and listed on the state Advanced Practice Nurse (APN) Registry required. Board certified as an Advanced Practice Registered Nurse, in applicable population foci and/or subspecialty.
  • RN License required
  • BLS from the American Heart Association may be required depending on practice setting
  • ACLS from the American Heart Association may be required depending on practice setting
