Job Description

ACT is an evidence-based practice, interdisciplinary team-based approach to the provision of treatment for individuals with severe mental illness and often co-occurring substance use disorders. Therapeutic interventions focus on promoting symptom stability, increasing coping skills, social skill development and community integration. Services include wraparound care navigation to assist individuals with connection to community resources, skills training to help increase independent living, coordination of care with relevant supports, individual and family therapy, substance use counseling, medication management, psychoeducation, peer support, and vocational services

Must hold at least a LPC, LMFT or LCSW in the state of Colorado.

We are willing to provide supervision to anyone wanting to become a LAC.

Schedule: Monday - Friday 8AM - 5PM, hybrid working schedule

Purpose:

To assist people with mental illnesses, including those in dual recovery, in obtaining optimal functioning and quality of life, by providing clinical services and education with the emphasis within the ACT Model:

  • Specialized caseload which provides community based services to clients with severe mental illness and co-occurring disorders.
  • Collaboration with community partners to meet client specific needs
  • Provides intensive case management services both directly and by referral
  • Provides community based services to enhance skills and promote community integration
  • Provides therapeutic and case management services with the goal of assisting the clients movement to lower levels of care

Essential Functions:

  • Guided by 27-65 requirements and best clinical practices, provides individual and group psychotherapy and teaches psychoeducation
  • Documents provision of services, assessments, evaluations and treatment goals and plan consistent with clinical and administrative policies and procedures
  • Provides a variety of case management services, including managing Social Security payeeships, assisting clients to apply for various types of public assistance and providing supporting documentation, attending multi-agency clinical case consultations, and facilitating referrals to other organizations
  • Participates in internal consultation and supervision with team members and other professionals in the organization
  • Collaborate with clients, other treatment team members, DHHS, doctors, hospitals, medical clinics, housing programs, etc. to provide the best service to the clients
  • Provides external consultation and referrals to other organizations
  • Maintains contact with clients in distress, including assisting with hospitalization
  • Participates in multidisciplinary focus groups, committees, and task forces charged with researching and improving clinical practices and developing or updating programs and business processes
  • Provides clinical services required by special programs and projects
  • Meets defined individual and department goals, activity metrics and Key Performance Indicators
  • Promotes, supports and demonstrates MHPs mission, vision and values through both behavior and job performance and adheres to policies, procedures, Compliance Plan and Standards of Conduct on a day to day basis
  • Effectively interacts and communicates with other MHP staff/clients/customers/partners/etc.
  • Regular and consistent attendance is required to perform other essential functions of the job

Key Technical Skills and Knowledge:
Education: Masters degree in Social Work, Counseling, Psychology or related field.
Experience: Minimum of 2 years experience working in a clinical setting in the behavioral health field.
Demonstrate a high level of skill in assessment, evaluation and therapeutic engagement of diverse clients; use of evidence-based counseling models and techniques that match presenting problems; good diagnostic skills and treatment/care planning; and the ability to work with clinical documentation and electronic health records (EHR).
Knowledge of DSM V and working principles of recovery and trauma-informed care in treatment planning and in direct care. Experience working with individuals and families who experience various mental, behavioral, and substance abuse conditions.
Ability to operate PC-based software programs including proficiency in Word, Excel and other computer based systems.
Possess advanced written and oral communication, decision making and problem solving skills, and efficient time management. Excellent interpersonal skills, the ability to work independently and as part of a team; collaborating with other internal and external teams.
Must possess own transportation, have a good driving record, a valid drivers license, and proof of insurance.

Salary: 53,000 - $65,000 annually
