POSITION SUMMARY

This position is responsible for managing an assigned caseload of clients having long-term mental illness and for assessing client needs, developing, implementing and reviewing service plans, and working with other internal and external resources in meeting/achieving client service needs.

ESSENTIAL SKILLS AND EXPERIENCE:

  • Bachelors Degree in Social Service field is required or a minimum of five (5) years of behavioral health experience and one (1) year of direct experience may replace Bachelors Degree
  • High school diploma or GED from an accredited institution is required
  • Minimum of one (1) year of experience working with State AHCCCS and Regional Behavioral Health Authority (RBHA) clients; preferred
  • Excellent customer service skills
  • Excellent computer skills
  • Knowledge of Arizona Department of Health Services and RBHA requirements
  • Thorough knowledge of current versions of DSM and ICD
  • Ability to complete a Behavioral Health Assessment with appropriate clinical oversight
  • Ability to provide effective and respectful quality care and services that are responsive to diverse cultural beliefs and practices
  • CPR and First Aid certified
  • Bilingual English/Spanish strongly preferred
  • Valid Arizona drivers license
  • Level 1 Fingerprint clearance card required

POSITION REPONSIBILITIES/ESSENTIAL

  • Participates as directed in the screening of new service requests within the center ensuring that all individuals are adequately and appropriately served according to their individual needs and in accordance with existing policies and procedures
  • Participates in regular interdisciplinary staff meetings
  • Accepts newly assigned cases in which the diagnostic intake has been completed by the Intake Specialist. Completes case management assessment on all clients entering caseload
  • Develops appropriate treatment/service plans with clients as the means for implementing appropriate service and developing effective alliances with clients
  • Provides direct client services to individuals and/or family members. Services provided may include case management, skills training, group counseling, group skills training, health promotion, crisis intervention, transportation, supportive employment services, counseling with appropriate clinical oversight and others depending on client need. Services may be provided in office, home, school or community settings
  • Provides on-going supportive and/or case management functions in accordance with the problems, needs and the strategies identified within the service plan in order to assist the client to achieve goals and objectives
  • Provides face-to-face reviews with the client and/or relevant behavioral health staff on a regular basis. Ensures progress made is reported so that the service plan can be modified as necessary to ensure that the goals and objectives are being achieved. The frequency reviews will be determined by relevant requirements
  • Assist supervisory staff with tracking of clients, including the generation of reports that will allow follow-through on quality of care
  • Documents all service contacts on a timely basis including face-to-face interviews, collateral and networking contacts, correspondence and maintains the case records in accordance with internal and regulatory standards requirements
  • Maintains close communication with the consulting psychiatrist and the Behavioral Health team for input regarding medication compliance, side effects of medication, and medication changes and alerts behavioral health team of any changes in client adjustment which might suggest decompensation and a need for more aggressive intervention
  • Performs other duties as assigned by the supervisor which are consistent with the position and in compliance with Adelante Health Care policies and procedures

Work environment: While performing the duties of this job, the employee is exposed to weather conditions prevalent at the time when traveling to various clinic sites. The noise level in the work environment is usually moderate. The employee may be subject to health hazards (contagious diseases, blood borne pathogens, etc.) when working in the clinic area.
