Our client is seeking a Human Services Specialist to engage youth and their families in intensive care coordination, case management and support services. Their role is to identify youth referred by human service agencies, schools, or the court system; and are at risk of placement outside the home (i.e. foster care, psychiatric hospital, detention, or residential treatment) or are in congregate care preparing for return to the community.

This position is currently fully remote and will eventually return to the office in Alexandria, Virginia.

SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY
* Facilitate High Fidelity Wraparound care coordination and provide other intensive care coordination services (planning and monitoring services for children in and at risk of residential treatment, plan for discharge);
* Provide community and office-based interventions that address the unique potential and needs of youth and families in support of treatment plan goals and objectives, including care during crisis
* Provide administrative intakes and follow-up with intake contacts
* Participate in multi-disciplinary treatment planning meetings, court hearings, and relevant training, conferences and workshops
* Collaborate and coordinate closely with child welfare and other stakeholders within the system of care, i.e. family support partners, juvenile justice, schools etc.
* Collect outcome research and satisfaction data with persons served
* Maintain medical records in accordance with accreditation body, Department policies, licensing and Medicaid regulations

REQUIRED EXPERIENCE AND EDUCATION
* Bilingual (English/Spanish) skills to meet needs of persons served
* Minimum of a Bachelor's Degree in Human Services or Special Education from an accredited college with at least one year of clinical experience with children and adolescents.
* Successful completion of Federal and local criminal records check and CPS check
* Ability to work some evenings
* Meet criteria to become a Qualified Mental Health Professional-Child (QMHP-C)
