Compensation (Pay): $13.61 to $15.66 based on experience.

Job Summary:
The Developmentally Disabled Health Center (DDHC) Care Coordination Navigator is responsible for ensuring effective coordination of care for patients of the DDHC clinic with external agencies including those contracted by the state to provide services for patients with a diagnosis of developmental delays. This position is also responsible for monitoring communications with referring community partners, closing care gaps, and maintaining patient records. The goal of this position is to promote Patient Centered Medical Home concept by providing a centralized and resource for care and agency coordination which is fully integrated with the primary care team. This DDHC Care Coordination Navigator is responsible for many aspects of patient needs and customer service such as providing patient education and outreach on the continuum of health care services offered throughout the community; foster and build relationships with many collaborative partners to include, but not limited to: The Resource Exchange (TRE), The Department of Health Services (DHS), The El Paso County Department of Health and Environment (EPCDHE), private medical practices, local hospitals, urgent care and schools to ensure continuity of care to serve as a liaison and link between health and social services to facilitate access to primary and specialty healthcare services. They act as a liaison between the patients, providers, programs and resources available to the DD population. The DDHC Care Coordination Navigator is also responsible for acting as a liaison to other external agencies and ensuring monthly meeting (in person or virtual) is held in order to share available resources with community partners to improve the overall quality of care available throughout our community.

Job Duties:

  • Acts as a liaison between patients who qualify for services through TRE and other applicable external agencies by communicating patient needs with service coordinators or agency representatives in real time for DDHC patients.
  • Screen patients for eligibility of CICP, CHP+, Medicaid, and/or verifies private insurance. Assesses patients for barriers to care and assists them in overcoming the barriers in order to meet their on-going health care needs including assisting patients with waiver programs in cooperation with TRE.
  • Communicates and collaborates with other service providers and community resources in an effort to ensure holistic, continuity of care for the patient including the organizing and execution of a monthly DDHC "agency meeting."
  • Provides patients with appropriate community referrals and assists patients with communication to available resources when appropriate.
  • Seeks professional opinions and input from the appropriate providers and/or care teams.
  • Works with payer partners to close gaps in care or data if/when applicable.
  • Works in conjunction with the DDHC Resources Navigation team to ensure referral navigation as needed.
  • Completes other duties as assigned.

Qualifications:


Education: High school Diploma or equivalent required.
Work Experience: 1 year of customer service experience required. Experience in documenting health records, working with payers and including Medicaid waiver programs is preferred.
Cognitive Skills: Excellent oral and written communication skills, ability to understand complex issues. basic math skills required.
Computer Skills: The ability to type and proficiency with Microsoft Office (i.e Outlook, Word, and Excel) and internet browsers is required.
Other Skills: Conflict resolution, attention to detail, knowledge of community resources, confidence, and creativity required. Ability to work both as a team and independently, bi-lingual preferred, healthcare knowledge (e.g. payor sources, etc.) preferred.

Physical Demands: Frequent standing and walking, occasional long periods of sitting, moderate bending, moderate energy requirements, and frequent fine motor skills. Viewing computer monitors over an extended period; hearing within normal range; and must be able to lift 20 pounds.
