Job Description

The Case Manager is a professional case management position that is a member of a Co-Responder team consisting of a behavioral health clinician and a sworn officer of the Tampa Police Department (TPD). This position will include focus on individuals identified as high utilizers of crisis stabilization units, emergency rooms, and calls for police service related to behavioral health issues.

Essential Functions:

Coordinate admissions referred by field therapists, TPD, or other Co-Responder Team (CRT) representatives

Assess and/or screen referred individuals for admission criteria to ACTS addiction receiving, outpatient or residential services.

Maintain contact with those on caseload for a minimum of six months providing a comprehensive continuum of services to promote recovery. This timeline can be extended based upon the need of the individual.

Ensure smooth transitions to the next level of care (warm hand-offs), is completed, to avoid gaps in service, and respond rapidly for needed support and/or intervention.

Support individuals receiving services with integration back into society as a healthy, substance-free individual.

Facilitate appropriate connection and linkage with peer support specialists, education, employment, housing and/or other needed wraparound services.

Assist with the Opioid Task Force Overdose 24/7 Call-Outs as reasonably needed

May transports clients and/or materials to medical, legal, recreational and other community appointments that support their recovery.

Attend multidisciplinary team meetings to review case status

Attend court hearings, as needed

Prepare documents including, but not limited to, progress reports

Pre-requisite Requirements:

Bachelor's Degree in a human services related field

One (1) year of case management in a related experience.

Must possess valid Florida Driver License

Must be able to fully participate in Welle (crisis intervention) and CPR/First Aid training
