Job Title: Care Coordinator for Type 1 Diabetes and First Three

Reports To: Care Coordination Supervisor / Clinical Research Manager

Job Summary: The position will integrate the roles of coordinating for the First Three program and Type 1 Diabetes program. There will be some overlapping components that will be associated with both programs, but overall, these are two separate jobs that will be combined into one full time position.

Responsible for assisting and coordinating the clinical care for the Type 1 Diabetes Program and First Three Program.

Type 1 Diabetes Program Essential Job Duties:

Patient Enrollment/Tracking:

  • Perform intake on newly diagnosed or new patients to CCHCI with the diagnosis of type 1 diabetes.
  • Track patients in the program and to what extent he/she participates in the elements of the program.
  • Maintain an up-to-date spreadsheet of existing patients with the most recent visit with CCHCI provider and endocrinology specialist (if applicable and able)
  • Call and schedule patients with CCHCI provider for monthly diabetes follow-up visits.
  • Assist in recall efforts for gaps in care related to patients with diabetes.

Care Coordination:

  • Process and follow-up on orders and prior authorizations for medications and diabetes technology.
  • Help schedule and coordinate trainings for patients with new diabetes technology.
  • Help schedule and coordinate joint visits with endocrinology specialists.
  • Ensure that the specialists notes on file are up to date and that our follow-up notes are sent to the appropriate endocrinology specialist.

Community Engagement/Program Development:

  • Help schedule and perform outreach for quarterly support group meetings and fundraising efforts.
  • Help coordinate, schedule, and provide diabetes education sessions for interested school districts or healthcare training programs in Cochise County.
  • Assist families in establishing/maintaining home downloading capabilities for diabetes technology.
  • Assist with applying for grants and certifications when available.
  • Assist in the coordination of research efforts related to the program.
  • Assist in publicity and public relations related to the program.

First Three Program Essential Job Functions:

Patient Enrollment/Tracking:

  • Check the EPM list multiple times per week for newborn infants who are being scheduled in Douglas, prioritizing these patients First Three Providers
  • Assist with ensuring that families of newborn infants are being scheduled with First Three providers of the same language. Thus, if the patient speaks Spanish, they will schedule with Spanish-speaking First Three providers.
  • Update the Google Drive for First Three as new patients enroll in the First Three Program.
  • Work directly with the First Three research coordinator to assist in updating i2i with new patients (First Three and non-First Three).
  • Work directly with the First Three research coordinator to assist in updating i2i with new patients (First Three and non-First Three).
  • Assist in creation of First Three handouts, including translation, and ensuring that handouts are printed and updated for provider use at ECCE and PCE.
  • Ensure that First Three program has updated materials printed and ready for provider use (handouts, templates, folders, giveaways).

Care Coordination:

  • Organize First Three supplies, including books, toys, timers, etc. for each visit.
  • Communicate with each First Three provider on a weekly basis to ensure patient list is updated and answering their questions.
  • Schedule meetings for and communicate with the different home visitation organizations every other month, then helping to run the meetings.

Community Engagement/Program Development:

  • Assist in the ongoing training of new First Three providers.
  • Assist in the creation of a First Three-expansion pack, so that other clinics will be able to implement First Three into their clinical flow.
  • Assist in First Three publicity and public relations, including binder design, website building, video taking, YouTube page.
  • Creating and organizing a Family Action Board (aka. Family Advisory Board) for the First Three program. This includes contacting parents, inviting them to attend, organizing attendance, taking notes, and assuring that tasks are completed.
  • Assist with consenting of patients for research.
  • Advising and helping to lead First Three around community engagement, participation, and feedback.
  • Ensure that the community has a voice in the creation and expansion of the program.
  • Constructively participate in growth of community engagement around early childhood, including possible creation of a community liaison and Community Advisory Board.

Required Education, Experience, Certificates & Licenses:

  • High School Diploma or GED.
  • Minimum of one year of experience in health-related field.
  • Any combination of education and/or experience that provides the necessary skills and sensitivity.
  • CPR may be required.
  • Valid Fingerprint Clearance Cared required.

Required Language Skills:

  • Ability to comprehend and compose instructions, correspondence and communications in English and Spanish in both oral and written format.
  • Bilingual in English and Spanish with knowledge of medical terminology is required.


Physical Requirements:

  • Ability to frequently exert enough force to move objects weighing up to 25 pounds.
  • Ability to continuously remain in a stationary position.
  • Ability to frequently move about inside the workplace to assist patients, operate office or medical equipment, etc.
  • Possesses hand-eye coordination and manual dexterity necessary to constantly operate computer, telephone, and other office machinery.
  • Possesses close visual acuity necessary to accurately record and view information on a computer monitor, handwritten and typed documents.
  • Ability to discern the nature of sounds at a normal spoken volume.

Other Required Knowledge, Skills, and Abilities:

  • Ability to add, subtract, multiply and divide in all measure, using whole numbers, common fractions, and decimals.
  • Ability to gather data in an organized fashion from varied sources.
  • Ability to perform a variety of assignments with little or no direct supervision.
  • Ability to identify and resolve problems in a timely manner.
  • Knowledge of health plans and community health centers preferred.
  • Knowledge of HIPAA rules and regulations.
  • Computer literacy required with proficiency in use of all Microsoft Office programs.
  • Ability to provide excellent customer service by mail, telephone, and in-person.

Work Environment & Conditions:

  • Work environment is typical of a health clinic setting with occasional exposure to communicable diseases, bodily fluids, and hazardous chemicals.
  • Occasionally, work requires extended hours to include early mornings, evenings, and weekends.
