Supports the care manager and the comprehensive coordination of medical services including Care Team Intake, screening and supporting the implementation of care plans to promote effective utilization of healthcare services. Promotes/supports quality effectiveness of Healthcare Services.
Required Qualifications
Ability to support the comprehensive healthcare management, to facilitate delivery of appropriate quality healthcare, promote cost effective outcomes and improve program/operational efficiency involving clinical issues.
Effective communication, telephonic and organizational skills. Strong customer service skills to coordinate service delivery including attention to members and resolution of issues to promote positive outcomes for members.
Ability to effectively participate in a multi-disciplinary team including internal and external participants.
Preferred Qualifications
Promotes/supports quality effectiveness of Healthcare Services.
Identifies members for referral into Aetna's Case Management, Foster Care and other specialty programs.
Computer literacy in order to navigate through internal/external computer systems and demonstrates proficiency in Microsoft Word and excel.
Effective communication skills, both oral and written.
Education
High School Diploma or GED
