This position will be working from home with potential to move onsite once COVID restrictions are lifted. Travel will be required once COVID restrictions are lifted, and the employee will be expected to be in the Arlington, TX community and surrounding areas conducting home visits up to 50-75% of their workdays.
Utilizes critical thinking and judgment to collaborate and inform the case management process, in order to facilitate appropriate health care outcomes for members by providing care coordination, support and education for members through the use of care management tools and resources.
Required Qualifications
Case management and discharge planning experience.
2 years experience in behavioral health, social services or appropriate related field equivalent to program focus.
Preferred Qualifications
Managed Care experience preferred.
Education
Bachelor's degree or non-licensed master level clinician required, with either degree being in behavioral health or human services preferred (psychology,social work, marriage and family therapy, counseling).
