    DEFINITION:
    Reaches out, provides resources and support and advocates for TAPP students/families in the Sunnyside School District Sunnyside, Desert View, STAR and middle schools. This individual should self-directed who can function with minimal supervision.

    ESSENTIAL FUNCTIONS:

    TASKS:
    Assists with registration and intake of new students; participates in on-going evaluations of student needs; reaches out to hard-to-reach students for services; coordinates community referrals as necessary; participates in case management staffing; evaluates and coordinates services to male partners and student families; focus is on reducing absenteeism; provides personal and social service counseling/advocacy; assists in coordination of Gov. services: AHCCCS, WIC, Food Stamps, DES, AFDC; maintains contact and follows-up with TAPP students; pregnancy prevention; senior grade/grad checks; prepares newsletter for the TAPP Program; performs other duties as assigned.

    KNOWLEDGE, SKILLS AND OTHER CHARACTERISTICS:
    Knowledge of District policies and procedures.
    Knowledge of office equipment.
    Knowledge of community resources and outreach organizations.
    Skill in following oral and written instructions.
    Skill in establishing and maintaining effective working relationships.

    PHYSICAL REQUIREMENTS: Work is performed in an indoor / outdoor environment. May be exposed to extreme weather conditions, toxic fumes, and hazardous and bio-hazardous materials. Work requires frequent walking, bending, sitting, vision to monitor and lifting up to 25 pounds.

    QUALIFICATIONS:
    Bachelors Degree in Social Work or a related field.
    Two (2) years of experience in working with social service agencies in the community, Experience with evaluation of school age children required.
    Spanish bilingual preferred.

    Salary Grade: PNT Grade 5; $35,019 - $49,036 FTE: 1.0 Days: 211 Start Date: 2021/2022 School Year
