Characteristics of Position:


COMPREHENSIVE OPIOD ABUSE SITE-BASED PROGRAM

This position providesassessment for mental health and substance use issues, basic integrated andsupported individual, family and group education, advocacy, linkage,coordination, problem-solving and monitoring of clients to optimize thefunctioning of individuals with substance disorders or co-occurring substanceand mental disorders and promoting stabilization, rehabilitation and recovery.

LOCATION: Tampa, FL

Pre-requisiteRequirements:

Bachelor's in human services or other relevantfield and one year of case management or related experience.

Quantity of Work Expected: Commensurate with levelnecessary to fully meet obligations

Supervision: Work is performedunder the direction of the Program Supervisor, Director or Administrator. Thisposition does not provide supervision to others.

Scopeof Responsibility: Conducts interviews to assess social, legal,medical psychological and educational needs of client; provides caseworkservices to assist clients in obtaining social, educational and vocationalgoals.

Engages clients to talk and communicatesempathically about their addiction or mental illness, medications and recoveryto promote understanding of their illness/illnesses and hope for recovery.

CoreResponsibilities/Essential Functions:

After contact and initial screening is made by the Opioid Task ForceOfficers/Detectives, Case Manager may assist to complete an initial screening andcoordinate obtaining detox services and/or a full biopsychosocial assessment.

Maintain contact with the individual for a minimum of six monthsproviding a comprehensive continuum of services topromote recovery and enabling the substance abuse individual referral to fullyintegrate back into society as a healthy, substance-free individual. Thistimeline can be extended based upon the need of the individual

Ensure smooth transitions to the next level of care (warmhand-offs), is completed, to avoid gaps in service, and respond rapidly forneeded support and/or intervention.

Facilitate the appropriate connection with peer support specialists,education, employment, housing and/or other needed wrap around services. The Substance Abuse Program Case Manager willspecifically assist with the Opioid Task Force Overdose 24/7 Call-Outs asreasonably needed

Complete admission and orientation documentation; may administeror interpret screening/assessment instruments.

Document all clients' contacts, care and response, maintainrecords and updated rosters collect data and prepare reports as mandated byprogram procedure within agency standards and timeframes.

Attend and serve as advocate for client in court hearings asapplicable.

Maintain a comprehensive listing of community resources,including social, legal and educational providers.

Attend scheduled interdisciplinary treatment team meeting orclinical supervisory sessions and discusses aspects of client's care andprogress (or lack of same); may participate in other meetings/events asassigned.

Recognize and promptly/appropriately assist as needed incrisis situations to minimize incidents of hospitalization or incarceration;report treatment needs of clients to clinical personnel and supervisor;recognize client's comprehensive needs and discusses with interdisciplinarytreatment team.

Complete documentation including but not limited tobehavioral progress reports, incident reports, grievances, injury reports,client eligibility, enrollment, tracking, lease agreements, billing and relateddocumentation as required.

At times may beasked to ride with Opioid Task Force Officers/Detectives during SpecialOperational Plans involving potential individuals in need of services

Perform related work as requested / required.
