Department: Prevention Intervention and Behavioral Health

Salary: $22.25 Hourly
FT/PT Status: Reg Full Time

Summary:

  • Must be 21 years of age.
  • Previous experience with youth who have experienced trauma to include family violence, community violence, or sexual assault. Also has knowledge or experience working with or supporting DMST (Domestic Minor Sex Trafficking).

Job Description:

CASE MANAGER II SAFE HOUSE: Under the direct supervision of the Program Manager. Will organize and coordinate case management services to clients to create a safe, supportive, trauma-responsive living environment and provide related services to Domestic Minors Sex Trafficked (DMST) survivors ages 14-18 years old.

MINIMUM QUALIFICATIONS:

  • Bachelors degree in social work or related counseling field of study and two (two) experience providing case management services to children and families; or six (6) years experience providing services to children, youth, and families, and/or a combination of Bachelors degree in social work or related counseling field of study and two (2) years experience providing case management services to children and families; or six (6) years experience providing services to children, youth and families, and/or a combination of education and experience to equals six (6) years.
  • Must be 21 years of age.
  • Previous experience with youth who have experienced trauma to include family violence, community violence, or sexual assault. Also has knowledge or experience working with or supporting DMST (Domestic Minor Sex Trafficking).
  • Extensive knowledge of community resources and the needs of youth with complex trauma history.
  • Ability to complete assessment, service planning, and service coordination with high-risk clients.
  • Ability to establish strong working relationships with the public, especially with the staff of community agencies.
  • Ability to exhibit emotional maturity and good judgment, including high morals and speech standards.
  • Must be dependable and flexible with regard to working hours.
  • Ability to follow and carry out written and oral instructions quickly and accurately.
  • Must possess basic computer skills.
  • Must be able to use discretion in all facets of work-related situations.
  • Ability to perform all work rapidly and accurately, meeting all deadlines when applicable.
  • Must keep abreast of modern techniques and methods used in individual and group sessions.

DUTIES AND RESPONSIBILITIES (Not Inclusive):

  • Will schedule and complete client intakes and comprehensive case management needs assessment.
  • Will develop a crisis/safety plan with all clients.
  • Will develop a service plan to identify all areas of services to be provided, including goals and objectives for each service.
  • Will participate in weekly supervision with the Program Manager and attend monthly group supervision.
  • Will facilitate access to services for clients in the areas of medical, social, educational, vocational, legal, and therapeutic care.
  • Will work cooperatively and collaboratively with the assigned Clinical Therapist.
  • Will monitor and evaluate the provision of client services on an ongoing basis.
  • Will provide advocacy and referral services on behalf of clients.
  • Will provide crisis intervention services.
  • Will provide life skills, parenting, and conflict management training.
  • Will provide after-care planning and follow-up services for all clients discharged from the program.
  • Will assist in training of program staff.
  • Will obtain the necessary medical paperwork for each program youth.
  • Will assist the program manager in overseeing and monitoring case records and implementing the use of the recordkeeping system by staff.
  • Will assist in scheduling medical appointments.
  • Will maintain weekly case management documentation for each program youth, comprehensive client records, and generate reports as required.
  • Adhere to the agency Program Policies and Procedures Manual, Program Operations Manual, handbooks, and Personnel Policies and Procedures Manual.
  • Will assist in transporting program youth during intake, outings, activities, and scheduled appointments.
  • Will participate in weekly staff meetings.
  • Must adhere to the Trauma Responsive Care and Services Training Requirements, attending required training as scheduled.
  • Will complete critical incident reporting as necessary and provide support to staff in reviewing and signing off on reporting.
  • Will rotate on-call procedures to ensure staff-to-client ratio.
  • Shifts are subject to change due to program needs, and case manager may be required to cover shifts.
  • Maintain a neat, organized, and clean work environment.
  • Develop and maintain structure, rules, and consequences for program youth.
  • Assist the Program Manager in developing and maintaining program requirements.
  • Other duties as assigned.

JOB CHARACTERISTICS/PHYSICAL DEMANDS:

This is a professional position which requires organizational and communication skills, as well as judgment, tact and diplomacy in contacts with YDI employees, community residents, parents and local government agencies. Strict adherence is required to all regulations concerning confidentiality. Background investigation to including fingerprinting, is required. Physical is required upon offer of employment to include evidence of COVID vaccination. Updated physical and vaccination may be required. Frequent travel is required by employees, valid drivers license, good driving record, and proof of satisfactory car insurance is required.
