Our client is seeking a Case Manager in Mesa, AZ.
SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY
* Perform intermediate professional work assessing client needs, developing, implementing and monitoring service delivery and assistance plans, coordinating and monitoring services with other agencies.
* Counsel and assist clients, maintaining records and files, preparing reports, and related work as apparent or assigned.
* Plan and facilitate group sessions with clients participating in community activities and/or to promote recovery and educate clients.
* Provide crisis intervention and prevention services.
* Attend court hearings; testifies in court.
* Assist in curriculum development and resource tools.
* Thorough knowledge of community and agency resources and programs of regional and state sources for the community service population
* Family and group Therapy
REQUIRED EXPERIENCE & EDUCATION
* Bachelor's Degree in Social Work, Psychology, Counseling, Sociology, Health Services or related field
* Must be QMHP-A certified
* State Background Check
* Valid Driver's License
* Moderate experience working with individuals with behavioral health disorders and/or intellectual disabilities in a behavioral health setting, or equivalent combination of education and experience
