Case Manager of Behavioral Health Services (FWU)

Summary

This position is responsible for managing an assigned caseload of adult and children clients having mental health illness for accessing client needs, developing, implementing and reviewing client service plans, and working with other community resources in meeting/achieving client service needs. The FWU case manager provides individual case management and follow up services for each client. While working directly with the clients, the Case Manager is required to have knowledge and understanding of issues, including issues pertaining to special education, the IEP process, San Diego Regional Center Services, linkage to county services including services for severely mentally ill individuals and substance dependence and abuse service, homeless services, and domestic violence services. The case manager is responsible for providing clients with appropriate community resources and program development. This position reports directly to the FWUs Behavioral Health Director and coordinates daily operations with the manager as needed.

Responsibilities

  • Accepts newly assigned cases in which the diagnostic intake has been completed by a licensed provider. Completes case management assessment on all clients entering caseload.
  • Develops appropriate treatment/service plans with clients as the means for implementing appropriate services and developing effective alliances with clients.
  • Provides on-going supportive and/or case management functions in accordance with the problems, needs, and the strategies identified within the service plan in order to help the clients achieve the stated goals and objectives.
  • Documents all service contacts on a timely basis including face-to-face interviews, collateral and networking contacts, correspondence and maintains the case records in accordance with agency and regulatory stands and requirements.
  • Meets regularly with the immediate supervisor as a means of enhancing professional growth, reviewing and processing the provision of case management services, and dealing with appropriate administrative issues. .
  • Assure clients adherence to the policies and procedures of the program and perform appropriate disciplinary measures for non-compliance.
  • Works in conjunction with other program staff to identify areas to be addressed in the case plan.
  • Provides clients coordination, referral and linkage to appropriate services. .
  • Attends IEP as requested by the program.
  • Assist with special projects and/or assignments at request of supervisor.
  • Attend meetings as required and participates in committees as directed by the FWU Behavioral Health Director and the management team.
  • Ability to communicate effectively and respectfully with people of diverse lifestyles, backgrounds and ethnic and cultural groups.
  • Ability to use electronic health records accordingly.
  • Ability to use Microsoft Office and other essential computer programs.

Job Requirements

  • Must possess valid California drivers license and appropriate auto insurance as required by law
  • Must have a working knowledge of clinic operations, social services and community resources

Education Required (Minimum level of education):

Bachelor degree in social work or related degree (counseling, psychology) preferred but not required.

Experience Required (Minimum Level of experience):

Requires at least 2 years of working in the mental health and/or community outreach field, and office skills.

Verbal and Written Skills to perform the job: Bilingual (English/Spanish). Familiarity with basic word processing, spreadsheet, and database applications. Accurate keyboarding skills.
