We have an immediate need for a Case Manager - RN. This position coordinates access to appropriate quality health care services, while promoting the delivery of cost-efficient services that meet patient, provider and payer demands. Participates in discharge planning. Helps to identify changes/trends in practice patterns and assists in collecting and analyzing this data. Serves as a member of the interdisciplinary team for patient care.

Hours:

• 28 hours per week 1st Shift (7:00am - 3:30pm)

Qualifications:

• Graduate of an accredited school of nursing, and current unrestricted Wisconsin RN license required. BSN strongly preferred.
• A minimum of 3 years of nursing experience required, hospital nursing experience preferred. Experience in utilization review/discharge planning, case management helpful.
• Excellent communication, interpersonal, and problem solving skills required. Must have ability to manage multiple priorities and have knowledge of community resources.
• CPR certification required.
