Our client is currently URGENTLY seeking a Case Manager to assist with long term independent contract schedule needs located in Camden County.

Minimum Requirements: BA/BS in Human Services Field and 1 year work experience.

Provide case management and assessment both onsite and off-site settings; assures coordinated service delivery and successful client linkage with other programs and facilities; related duties as assigned.
This position provides Case Management Services to consumers to help consumers gain access to resources and services needed to meet their mental health needs, achieve recovery, and live independently in the community. Services are coordinated with internal and external providers to help each consumer achieve personalized case management needs.

Responsibilities:
1. Provide assistance, referrals, and care coordination in the areas of housing, applying for welfare, Medicaid, supportive education, supportive employment and other needed services.
2. Maintains a caseload of case management consumers.
3. Responsible for documenting case management progress notes in a timely fashion.
4. Completes initial applications of referrals on a scheduled basis.
5. Completes documentation of all initial applications within 8 hours.
6. Provide outreach for consumers that have missed their appointments.
7. Attend multi-disciplinary team meetings to provide input and obtain feedback about consumer cases.
8. Develop community contacts to secure community referrals to ensure consumers needs are met.
9. Utilize person centered approach to aid consumers to meet their prioritized needs.
10. Attend trainings as appropriate to the position or for requirements for the agency to maintain positive open and respectful communications with consumers, families, and other involved persons.
11. May require group facilitating, assessment, or log reporting of consumers.

CLIENT'S MINIMUM REQUIREMENTS,
*BA in Human Service Field (preferred).
*1 year post degree relevant work experience.
*Organized and demonstrates strong leadership and interpersonal skills
*Ability to work with computers and necessary software
*May require vaccination with booster

Compensation:
*Rates based on expertise, experience, and credentials

Experience: post degree related mental health experience: 1 year (Required)

License/Certification: Driver's License (may be required)
