Job Description
Southwest Behavioral and Health Services is seeking a dedicated and empathetic Children Case Manager who can help us fulfill our mission. Delivering compassionate care to enhance lives and improve communities. We are seeking someone who enjoys working with children, ability solve problems, organized and excellent communication skills. We are now offering a sign-on bonus of $1500 upon hire. Eligible for $1,000 after your first 60 days and $500 eligible to be received after 6months.
Job Preview at a Glance:
The Children Case Manager serves as a central support within the interdisciplinary team. This staff member conducts assessments, assists members with resources within the community and provides coordination of care inside and outside of the agency.
Pay Rate: Starts $17.50 hourly
Responsibilities:
Intake screenings (Completing intake paperwork and referrals)
Completing annual behavioral health assessments, including service planning and support planning
Facilitating CFTs and ARTs
Facilitating groups
Identifying gaps in care for our members
Completing SMI and SED evaluation
Qualifications:
- Minimum of 21-year of age
- Bachelors degree in the behavioral health field plus one year of relevant experience in behavioral health
- A combination of experience and education totaling 2 years is acceptable.
- Valid AZ drivers license & motor vehicle clearance.
- Must have fingerprint clearance issued by the Arizona Department of Public Safety or be eligible to receive within eight (8) weeks of hire.
