SALARY RANGE: CMII - $22.00/hr. - $26.42/hr. (Masters Degree)

The case manager II will provide therapy to children and families experiencing a high level of mental health needs. Services will consist of individual therapy, family therapy, treatment with parents, and case management with a range of trauma-informed interventions. Case management services will include a wide range of prevention, early intervention, and treatment services to community members. The case manager II will also develop points of entry to MHCDs Healthy Living Team and coordinate presentations, events, and specialized programming throughout the Denver area. Promote Child & Family Services as the provider of choice for supporting the health and well-being of children and families. The case manager II will also be a consultant to building staff and community partners. Responsibilities also include developing treatment plans and related documentation and collaboration with community partners. As a clinical case manager you will be providing transportation to the people we serve in your vehicle (additional cost for insurance is reimbursed).

ESSENTIAL JOB FUNCTIONS:

  • Provide intensive individual therapy, family therapy and crisis intervention in a community-based setting. Collaborate with internal and external partners in support of the familys case management needs. Provide advocacy when needed.
  • Provide strengths-based, family-centered, culturally competent, and community responsive prevention and early intervention group supports in an empathic, multi-disciplinary, comprehensive manner for children, families, and the community within greater Denver Metro area.
  • Assist clients with psychological development, social development, and rehabilitation.
  • Proficient and skilled working in an inclusive, culturally sensitive and reflective manner.
  • Must be comfortable with navigating and outreaching diverse populations within the Denver community and be able to create and support healthy relationships within the Denver Metro area.
  • Develop relationships with community-based organizations that result in appropriate referrals to MHCDs Healthy Living Team.
  • Arrange presentations to community-based organizations that increase the health and well-being and prevention literacy of community members.
  • Organize events at community sites that focus on the provision of mental health services to underserved populations (e.g. assessments and evaluations)
  • Must function effectively in a highly autonomous role requiring accurate clinical judgments of mental health issues
  • Maintain accurate and timely clinical records consistent with MHCD standards.
  • Participate in team/MHCD meetings, in-service as well as relevant community meetings.
  • Perform other related duties as assigned.

REQUIREMENTS:

EDUCATION: Master (CM II) Degree in psychology, social work, or other related human services degree. Working experience with communications, marketing, media or related field a plus.

EXPERIENCE: Minimum 2 years in the behavioral health field and experience with group facilitation. Experience with Health and Wellness programming and promotion a plus. Group facilitation and engagement skills is a must.

Must have valid Colorado Drivers license and a recent clean driving record. Ability to transport the people we serve in your vehicle. Ability to obtain and carry Business Use coverage on your auto policy (additional cost is reimbursed).

SKILLS AND COMPETENCIES:

  • Knowledge or ability to learn and practice trauma informed principles and practices.
  • Group facilitation and engagement/relationship building skills needed.
  • Knowledge of community resources.
  • Ability to assess crisis situations and intervene appropriately.
  • Knowledge of methods of psychotherapy and community resources.

  • Knowledge and understanding of trauma-informed care principles/practices.
  • Familiarity with the DSM V and diagnostic techniques.
  • Effective written and verbal communication skills.
  • Skilled at partnering with youth and collaterals to meet mental health needs.
  • Can work autonomously in community settings.
  • Must have valid drivers license and good driving record.
  • Spanish bilingual strongly preferred ($2.50/hr. Spanish Differential)

SUPERVISORY RESPONSIBILITIES: None

MACHINES AND EQUIPMENT TO BE USED: Computers, calculators, fax machines, copiers, telephone and a variety of other office/clerical equipment.

TYPICAL PHYSICAL DEMANDS: Requires sitting, standing, bending and reaching. May require lifting up to 50 pounds. Requires manual dexterity sufficient to operate standard office machines such as computers, fax machines, the telephone and other office and/or clinical equipment.

WORKING CONDITIONS: May require occasional evening or weekend hours.
