Job Profile

Job Summary:

In this role, Clinical Pharmacy Specialist with Internal Medicine will provide advanced clinical pharmacy services at Harris Health System. This individual will serve as a resource to the interdisciplinary team, and other pharmacists while delivering innovative practices and service delivery models that ensure cost effective and optimal medication management in the population we serve. Individual will participate in operational and clinical duties of a clinical pharmacy specialist and providing additional coverage where needed (e.g. Internal Medicine,)

Multi-disciplinary rounds, care coordination rounds including clinical initiatives, direct patient care and educational/training activities will be performed. Individual will also provide education to technicians, pharmacists, and other health care practitioners. They will serve as a preceptor of pharmacy interns, technicians, and ASHP-accredited pharmacy residents.

MINIMUM QUALIFICATIONS:

Education/Specialized Training/Licensure: Bachelors degree in Pharmacy Required, PharmD Preferred
+ Registered with Texas State Board of Pharmacy; If licensure by reciprocity or if new pharmacy graduate, must obtain Texas license within the first 90 days of employment. General Pharmacy Practice Residency Required

ASHP accredited PGY2 training or PGY1 residency program plus 2 years post-residency relevant clinical pharmacy experience.

ACPE IV Sterile Certification preferred

Board of Pharmacy Specialties Certification required

Work Experience: 2 years total pharmacy experience preferred
