Overall Job Objective

What Success Looks Like In This Job

Under the direction of the Collaborative Management Program (CMP) Interagency Oversight Group (IOG), chair, vice chair and executive committee, the purpose of this position is to facilitate and coordinate the work of the CMP and ensure that Adams County meets the requirements for House Bill 1451. This position was created to reduce duplication and fragmentation of services for children, youth and families involved in multiple systems or at risk of involvement in those systems. Provides direct supervision of staff and leadership/administration of the CMP. Assures that the mission, vision and objective of House Bill 1451 are effectively carried out.

Examples of Duties for Success

  • Maintains a thorough knowledge and understanding of the statutory requirements under Colorado Revised Statutes 24-1.9-101and 12 CCR 2509-1 Department of Human Services Regulations 7.303 and keeps the IOG apprised of Adams Countys obligations and expectations regarding the legislation and requirements.
  • Develop and maintain effective and efficient relationships with all collaborative management program partners and other community professionals through effective coordination, communication, and continuity.
  • Facilitates and implements the work of the CMP through planning processes to assist the group of community leaders in identifying priority issues and in drafting a strategic plan to accomplish its goals and objectives.
  • Organizes, facilitates, and participates in professional multi-system meetings and Individual Service and Support Teams (ISST) with partner agencies for collaborative programming for the children, youth, and families in Adams County.
  • Oversee the development and implementation of House Bill 1451 Memorandum of Understanding
  • Tracks the progress and provides written reports, data and presentations to the CMP summarizing the successes and challenges of the strategic plan to ensure that up-to-date, transparent, collaborative information is available.
  • Provides oversight and assistance to employees by assigning and monitoring their workload and work quality through the principles of Partnering for Success.
  • Represents Adams CMP at the Statewide Steering committee and acts as a liaison to the state for all matters concerning the program.
  • Gathers data, documentation, and research, prepares reports and information relevant to any of the program partners to further the mission of the CMP.
  • Coordinates data collection, analysis, evaluation and reporting as required.
  • Gather budgetary information from IOG partners and monitors and maintains accurate records of CMP income and expenditures to ensure CMP funds are administered as directed.
  • Maintains confidentiality related to CMP families and ensures all releases and data sharing agreements are accurate and up to date.
  • Manages data system, including but not limited to data collection, data entry, analyses, and reporting.
  • Prepares and submits annual report to the Colorado Department of Human Services.
  • Develops outreach materials and markets the CMP to increase awareness and utilization of services.
  • Ensures all quality assurance reviews and paperwork are completed accurately and within appropriate timeframes.
  • Serves as a liaison with community agencies, identifies barriers to implementation of family-centered, community-based model of service delivery and facilitated communication with and between all involved parties.
  • Accepts, screens and processes referrals to the CMP including conducting intakes or client management as needed.
  • Perform special projects and assignments as requested, organizes, and track complex projects and team member obligations.
  • Performs other duties as assigned.
Qualifications for Success

  • Proficiency in all Microsoft applications.

  • Establish, foster, and maintain a positive working relationship with the ten (10) mandated partners to the collaborative management program.
  • Knowledge of best practice consensus building approaches and methods, and ability to promote consensus among diverse opinions and be sensitive to cultural and ethnic diversity.
  • Ability to interpret and apply administrative and departmental policies, procedures, laws and regulations.
  • Ability and skill to adapt and respond to changing priorities.
  • Ability to respond to questions from the public, county personnel, system partners and community regarding policies, procedures, and the collaborative management program.
  • Ability to perform responsible and difficult administrative work involving the use of independent judgement and personal initiative.
  • Ability to analyze situations carefully and adopt effective courses of action.
  • Ability to work independently.

More Qualifications for Success

  • Experience Requirements:
    • Three (4 ) years of working within systems such as Child Welfare, Juvenile Justice, Education, Public Health or Mental Health.
    • A minimum of (2) years of supervisory or management experience in Child Welfare, Juvenile Justice, Education, Public Health or Mental Health.
    • Experience in conflict management, community partnerships or collaborative programming.
  • Education Requirements:
    • Bachelors degree in Human Services public health, criminal justice, or related field.
    • Facilitation training or completion within six (6) months of hire.

Additional experience may be substituted on a year-for-year basis for the bachelor's degree, but not for the required supervisory/management experience as detailed above.

  • License or Certificate:
    • Must possess a valid Colorado driers license with no current suspension, revocation or license denials
  • Background Check:
    • Must pass a criminal background check
