DISTINGUISHING FEATURES OF THE CLASS:
The Communicable Disease Investigator is responsible for providing community wide health promotion and
disease prevention services in the area of all infectious diseases by investigating. collecting patient information.
providing public health education and increasing community awareness regarding disease prevention to ensure
the health and wellbeing of Gila River Indian Community.

ESSENTIAL FUNCTIONS:


- Conduct communicable disease investigations on cases and their contacts through various methods such as
phone calls. postal mail. home visits, hospitals. detention centers, jails and other applicable areas.
0 Conduct partner services notifications on confirmed positive STD cases where applicable.
0 Provide health education to increase the community awareness regarding disease prevention. signs.
symptoms and the control measures.
0 Collect patient information and enter into the electronic database system (MEDSIS. CD database system);
and completes the Communicable Disease Report forms (CDR) and ensures the accuracy of these reports.
0 Assist the Communicable Disease Nurse (CDN) during an outbreak investigation with specimen collection,
completing the CDR forms and other required forms.
0 Assist the CDN with submitting completed CDR forms to the appropriate agencies.
0 Develop and maintain excellent working relationships with internal Tribal Health Department programs.
GRIC and GRHC departments; collaborates with State and other agencies.
0 Coordinate and participate with other programs/agencies to provide outreach at educational booths.
disseminate educational ?yers/brochures and other information regarding communicable diseases. signs,
symptoms, and prevention.
0 Assist and participate in any public health emergency activities and events.
0 Ensure patient confidentiality according to HIPAA and the Privacy Act.
0 Perform related duties as assigned.REQUIRED KNOWLEDGE SKILL AND ABILITY:0 Knowledge of communicable diseases including procedures and techniques used in identifying,
investigating, and preventing communicable disease.
0 Ability to make independent analytical decisions and must possess problem solving skills.
0 Ability to review and understand, HIPAA, Privacy Act, and GRIC Ordinances with regards to Disease
Surveillance.

- Ability and knowledge of Microsoft software: Excel, PowerPoint, Word, etc.
0 Excellent listening and communication skills;0 Ability to communicate orally and in writing to various audiences.

- Ability to establish and maintain effective working relationships with other employees, Community
Officials and outside local agencies.
0 Ability to perform all physical requirements of the position and lift up to 50 lbs.

REQUIRED EXPERIENCE AND TRAINING:
Associates degree from an accredited college or university in Social Science, Behavioral, Natural Science, or
health related field and a minimum of three (3) years work experience in public health, phlebotomist, Certified
Nursing Assistance (CNA) or other health related field.Bachelors Degree preferred.

ADDITIONAL REQUIREMENTS:
0 Required to a pass background check and fingerprint clearance as a condition of employment and must
continue to maintain throughout duration of employment. Background checks are required for positions that
involve regular contact with or control over Community Children in accordance with The Indian Child
Protection and Family Violence Prevention Act, 25 US. C. Sections 3201 through 3210 and C.F.R. Part 63
and positions that have regular contact with the Elderly.
0 Required to obtain a Tribal Operators permit. Valid state drivers license with a current proof of driving
record for the past 39 months will be required to qualify for a tribal driving permit. Proof of driving record
must be submitted with application and must not be more than 60 days old from the date of submission
