SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY
* Provide community-based case management services, primary, and behavioral health integrated services.
* Match specific supports and interventions to the unique needs of Individuals and recognize the importance of friends, family, and community relationships.
* Assess and monitor the health status of assignedIindividuals.
* Assist in the coordination and monitoring of medication for Individuals .
* Seek out, link, and collaborate with primary and specialty care physicians, community health agencies, and other health care providers.
REQUIRED EXPERIENCE & EDUCATION
* Current State of Michigan Registered Nursing license / Practical Nursing license
* Valid and unrestricted Driver's License
* Valid and unrestricted License to practice
* Transportation or automobile accessibility with current auto insurance.
