Position Summary |

Be a part of an innovative strategy to support students to succeed!Join UNM-Taos to lead our campus-wide case management system that provides UM-Taos students access to social,health, and material supports. We are seeking a qualified Lead Community Health Worker(CHW) to coordinate and supervise daily activities of a team engaged in providing outreach, education, case-management, systems navigation, and access to resourcesto program participants to improve their health and well-being. The position will also create program presentations, provide organizationaloutreach, and implement data tracking and analysis. The Lead CHW works underthe Student Support Services Department, along with receiving technicaldirection from our local Health Extension officer and the Student AffairsDepartment See the Position Description for additional information. |

Minimum Qualifications |
High school diploma or GED; at least 5 years of experience directly related to the duties and responsibilities specified. Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis. |
Preferred Qualifications |
A. Current Community Health Worker Certification. B. Ability to make administrative and procedural decisions and judgments on sensitive, confidential issues. C. Knowledge of community education agencies and resources and knowledge of multi-system outreach programs related to health care delivery, clinical education, and health-related services. D. Ability to plan, implement, and evaluate individual case-management care plan. E. Knowledge of social, material, and other barriers to care that may be encountered by clients. F. Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community. G. Employee development and performance management skills. H. Skill in use of technology, data management systems and tracking, and related software applications. I. Ability to communicate effectively, both orally and in writing.
