Job Description

As part of the Social Determinants of Health (SDOH) assist the Yuma County community members including Migrant Seasonal Farmworkers (MSFWs) navigate social and community services related to socioeconomic, education, employment, social support networks, and access to health care services by coordinating comprehensive outreach efforts and providing chronic disease self-management educational services.
Promotion of health programs through coordinated comprehensive and effective outreach efforts. To develop and implement educational/outreach programs to effectively meet the health care needs of Wellton, Yuma, Somerton and San Luis areas including migrant and seasonal farm workers. Schedule and manage appointments for patients with chronic disease. Document patient education and chronic disease related clinical data.

QUALIFICATION REQUIREMENTS:

  • Must be able to pass an annual Navigator training and obtain a Navigator license certification administered and approved by Centers for Medicare & Medicaid Services (CMS) as part of the state requirements to perform and process Health Insurance Marketplace applications.
  • At 90 days of employment employee must be able to complete 960 hours of paid experience to obtain a Community Health Worker license certification administered and approved by the Arizona Department of Health Services (ADHS).
  • Must be able to obtain a fingerprint clearance card. If not already obtained, Fingerprint Clearance card application must be submitted within 5 business days of employment.
  • Must be able to successfully pass a background check and a health and drug screen.
  • Must possess a valid driver's license.
  • To perform the job successfully an individual must be able to perform each essential duty satisfactory. The requirements listed below are representative of the knowledge, skill, and/or ability required.
  • Associate degree and or an equivalent of minimum three years of experience in public health community based programs.
  • Current CPR Certification
  • Must be bilingual in English and Spanish.
