One of our clients is seeking a motivated professional to work as a Case Manager Community Support in the greater Silver Spring area.

SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY
* Responsible for planning and providing community-based academic, vocational, social skills, and transition experiences to meet assigned students needs as outlined by county curricula and as stated in the students IEP.
* Under supervision of a certified Teacher, plan and implement lessons and activities designed to address the specific strengths and needs of students in order to meet the IEP goals.
* Provide instructional assistance to teachers.
* In collaboration with therapists and a Crisis Intervention Specialist, provide crisis management and assist students in accessing community resources.
* Support employment and career development including vocational assessment, career exploration, and Internship/employment placement.
* Maintain records of academic, behavioral, and vocational participation and progress on a regular basis.
* Maintain consistent communication with parents, employers, adult service providers and other outside agencies.
* Coordinate and provide transportation for assigned students between their places of residence, locations in the community for school-related activities.

REQUIRED SKILLS & EXPERIENCE
* Bachelors Degree in Social Work, Psychology, or a related discipline.
* Valid Drivers License required.
* Must meet State regulating agency driving requirements, maintain auto insurance and pass corporate insurance eligibility standards.
