Responsibilities of the HCBS Coordinator

  • Supervise a team of Direct Support Professionals.
  • Providing direct support care.
  • Provide assistance with living skills development, health maintenance, personal care, and behavioral development.
  • Report any concerns about health and safety and keeping documentation accurate and up-to-date.
  • Meet with people supported and their family members, and government representatives to create a plan to deliver services needed.
  • Complete necessary paperwork to\document services provided to people we support and to demonstrate that the program operates within state rules and guidelines.


Requirements of the HCBS Coordinator

  • High school or equivalent
  • Current Arizona driver license; Must qualify as driver for AIRES insurance
  • Must be at least 21 years of age
  • Must have the ability to successfully pass background check with Arizona Central Registry, Arizona Adult Protective Services and obtain Level One Fingerprint Clearance
  • Pass AIRES training
  • Good physical stamina and the ability to safely stand, pivot, and move up to 40 pounds in order to transfer people are required
  • Basic computer proficiency
  • Excellent communication skills with an ability to establish rapport with staff members and the individuals we support.
  • Strong organizational abilities
