Job Description

Summary:


The candidate will establish and develop relationships with external crisis services agencies, PERT, hospitals, and emergency rooms in the county of San Diego to connect patients with La Maestra’s full spectrum of medical, dental, substance use treatment, co-occurring disorder treatment services, and social services. The candidate will conduct an assessment with each patient to determine their appropriate level of care. The successful candidate will have a sincere interest in supporting patients in crisis, communicate effectively with others, possess excellent administrative skills, and pay close attention to detail.

Responsibilities:


• Establishes and develops relationships with external crisis services agencies, PERT, hospitals, and emergency rooms.
• Represents the department in a professional and congenial manner in person, in telephone conversations, and in written correspondence.
• Conducts assessments of patients in crisis to determine if they are in need of an outpatient level of care.
• Obtains all required documentation and releases of information from patient.
• Sets up appointments for patients with La Maestra’s medical, dental, substance use treatment, co-occurring disorder treatment services, and social services.
• Works in collaboration with internal departments and external agencies to ensure case management and patient status are communicated.
• Documents all patient information and interactions in a timely manner in the electronic health record.


Job Requirements:


Education: Bachelor’s degree, LCSW or LMFT preferred.
Experience: A minimum of two years of experience in related position required. Experience working with crisis services in the community required. Proficiency with Microsoft Excel, Word, PowerPoint and Outlook required.
Verbal and Written Skills to perform the job: Bilingual (English/Spanish) preferred. Familiarity with basic word processing, spreadsheet, and database applications. Accurate keyboarding skills. Technical knowledge and skills required to perform the job: Excellent verbal, written and inter-personal communication skills and the demonstrated ability to work with diverse individuals and groups. Demonstrate high levels of self-initiation and direction. Knowledge of communities we serve. Knowledge and experience working in medical settings and interacting collaboratively with medical teams. Skill in analyzing situations and making timely decisions.
Physical and Mental Requirements: Work involves sitting, talking, hearing, using hands to handle, feel or operate objects, tools, or controls, and reaching with hands and arms. The employee may be          required to push, pull, lift, and/or carry up to 20 pounds.
