Job Description

Summary:

The Diabetes Educator RN collaborates with other members of the healthcare team, patients, and families to consult on and facilitate diabetes education within the organization and the community. The Diabetes Educator RN focuses on consultations for patients that are newly diagnosed and/or have complex diabetes management needs and makes decisions guided by evidence based best practices, as well as policies, procedures, guidelines and standards.


Responsibilities:

Works on another's behalf and represents the concerns of the patient/family and healthcare staff; serves as a moral agent in identifying and helping to resolve ethical and clinical concerns within and outside the clinical setting; demonstrates compassion for the patient/families situation as evidenced by team/family feedback; demonstrates awareness of patient and family rights; applies knowledge of palliative care and end of life issues when aggressive, life-extending interventions are no longer desired; uses interdisciplinary resources; understands the purpose of the Bio-Ethics Committee and obtains ethics consultations appropriately.


Performs activities that create a compassionate, supportive, and therapeutic environment for patients and healthcare staff, with the aim of promoting comfort and healing and preventing unnecessary suffering; listens to patients and families; individualizes care and fosters therapeutic relationships; approaches patients and families with compassion, and offers emotional support; practices reflect recognition of the impact of family on patient outcomes; demonstrates understanding of the National Patient Safety Goals and impact to patient care; communicates effectively with peers, physicians, hospital departments, and other hospital resources to effectively manage patient care; identifies obstacles or barriers to communicating effectively with patients, families, and healthcare team; provides consultation for the care of diabetic patients utilizing evidence based practice; applies appropriate standards (AACE, ADA, Endocrine Society, etc.); assesses learning/self-care behavior; assesses medical/health/psychosocial and economic status implications; assesses current knowledge and self-management skills; collaborates with patient/family/care giver/healthcare team to develop individualized diabetes education plans, instructional methods, and behavioral goals; teaches/counsels regarding principles of care; evaluating, revising and documenting progress.


Two (2) or more years related experience


Education Required: Bachelors Degree in Nursing

CDE (American Association of Diabetic Educators)

RN- State Board of Nursing- AZ

Work Schedule: 8-hours, Days, Monday through Friday
