Summary

 

The Director of Nursing for La Maestra Community Health Centers is responsible for the development, implementation, and compliance of nursing policies, quality improvement efforts (in coordination with the Medical Director), the coordination of RN staffing, and professional development for the nursing staff of the organization. Duties include (but are not limited to) assisting the clinical nurse coordinators responsible for the day-to-day functions of the nursing and clinical staff; facilitating problem solving, designing and implementing professional development plans and programs, and assistance in the development and implementation of the Patient Centered Medical Home Model. The Director of Nursing is responsible for supervising, developing, and maintaining an environment that supports La Maestra’s Registered Nurses.

 

Responsibilities

 

  • Coordinates all nursing staff to ensure that effective nursing services are provided and quality standards are met
  • Develop and maintain an environment that supports the nursing staff by providing professional support, training, and supervision
  • Development, implementation, and compliance of nursing policies and quality improvement efforts. Monitors practice for effective implementation.
  • With input from Department Managers, Health Center Staff, and Executive Directors, design and implement new department programs and clinical offerings which reflect changing patient needs
  • Demonstrates and models a positive and professional demeanor
  • Coordination of staffing, including participation in the interviewing and hiring process of new RN staff
  • Coordinates and maintains the required training and skills of staff
  • Effectively and timely communicates pertinent information to direct reports and management staff
  • Monitors RN staffing levels of various nursing departments; and as necessary directs RN staff rotations and scheduling in order to meet increased or decreased nursing service demands
  • Actively supports, promotes, and works to fulfill the mission, vision, and values of LMCHC by: exhibiting an appreciation and understanding of the organizations patients and staff, ensuring a welcoming and inclusive environment for patients and staff through actions and manners, effectively communicating LMCHC’s vision and mission to staff and patients, demonstrating initiative and a positive attitude
  • Able to identify problems and develop feasible solutions within area of responsibility
  • Ensures availability and operation of necessary equipment and supplies for patient care
  • Ensures direct reports are current with all immunizations, mandatory trainings, certifications, CPR requirements, and other requirements
  • Ensures new employees are appropriately oriented into their department and LMCHC’s policies, procedures, mission, his/her duties, and other staff members
  • Ensures that all RN reviews are completed equitably and in a timely manner. Evaluates RN staff performance and competency by direct observation, patient care standards, patient and employee input, and other related means
  • When needed, implements employee corrective action measures
  • Effectively addresses and mediates personnel issues in order to promote a productive and healthy work environment
  • Is able to perform all Registered Nursing functions meeting or exceeding standards (see Registered Nursing Job Description and Competency Assessment)
  • Effectively uses all machines, equipment, forms, automated applications, computer programs (i.e. Nextgen, i2i Tracks, Microsoft Outlook), etc. as required in performing assigned work
  • Meets all mandatory training requirements and keeps current with professional licenses and certifications
  • Participates in annual and/or mandatory training activities
  • Ensures that security and safety protocols are enacted and followed for all supervised employees

  • Understands and demonstrates knowledge of different age groups and populations in order to provide effective, comprehensive care. Understands the normal growth and development for each age group, the different communication needs (changing communication methods and terminology accordingly), and understands the different safety risks for each age group and alters the patient’s care and environment accordingly.

Patient Centered Medical Home Essential Responsibilities (Principals)

 

·        Engages nurses in the successful adoption of PCMH

·        Works with department managers to train and educate staff to achieve adoption of PCMH concepts

·        Helps in the development of training and education around new systems of care delivery

·        Works with the staff to develop the established system of medical care utilizing a team approach

·        Collaborate with individual patients, their personal physicians, and when appropriate the patient’s family to ensure accessible, comprehensive, compassionate, and culturally competent care

  • Facilitate care by registries, information technology, health information exchange and other means to assure that patients get the indicated care when and where they need and want it in a culturally and linguistically appropriate manner.

·        Ensure all decisions respect patients’ wants, needs, and preferences and that patients have the education and support they require to make decisions and participate in their own care.

·        Continually strive to attend to each patients “whole person,” in the context of the patient’s personal and medical history and life circumstances, rather than focusing on a particular disease, organ, or system

 

Job Requirements

 

Education:  Bachelor of Science in Nursing from an accredited school of nursing.

 

Certification required: Current and active CA Registered Nurse License, current valid CPR/ACLS Certification

 

Experience: Five years’ experience in clinical nursing, preferably in a community health center setting; 2-3 years’ supervisory experience preferred.

 

Verbal and Written Skills to perform the job: Bilingual preferred. Familiarity with basic word processing, spreadsheet, and database applications. Accurate keyboarding skills.

 

Technical knowledge and skills required to perform the job: Excellent verbal, written and interpersonal communication skills and the demonstrated ability to work with diverse individuals and groups. Demonstrate high levels of self-initiation and direction. Knowledge of communities we serve. Knowledge and experience working in medical settings and interacting collaboratively with medical teams. Skill in analyzing situations and making timely decisions.

 

Physical and Mental Requirements: Work involves sitting, talking, hearing, using hands to handle, feel or operate objects, tools, or controls, and reaching with hands and arms.
