Director of Social Work
Troy Center is actively seeking a qualified Social Worker to be the Director for our Skilled Nursing Facility located in Troy, NY! Previous management experience is a plus but not required.

DUTIES:
- Directs, establishes & plans the overall policies/goals for the Social Services Dept
- Responsible for assessments, care planning & helping residents adjust
Coordinating discharges for the residents to the community.
- Occasional Community Outreach
- Familiar with a variety of concepts, practices & procedures
- A wide degree of creativity and latitude is expected
- Relies on extensive experience & judgment to plan and accomplish goals
REQUIREMENTS:
- Experience in LTC settings, case management, sub-acute rehab, discharge planning and working with vulnerable populations preferred
- MSW/BSW or related degree
- Exceptional interpersonal & leadership skills
- Demonstrating reliability and self-sufficiency
