POSITION SUMMARY:

Under the general supervision of the Director of Case Management, the Discharge Planner is responsible for assisting in the implementation of the Case Management Program to ensure ongoing assessment of patient care delivery according to established standards.

POSITION QUALIFICATIONS: Equal Opportunity Employer Minorities/Women/Veterans/Disabled

Current licensure in Texas: LVN required, RN preferred

Experience in health care delivery systems with focus on Case Management & Discharge Planning

Must have skills in effective communication and critical thinking ability to establish priorities, and creative problem solving. Able to work independently.

Excellent collaboration skills with family and interdisciplinary team members to facilitate discharge plan.
