Our Assessment Nurses conduct regular visits within clients homes. This position can be either a Licensed Practical Nurse or a Registered Nurse.

Specific Responsibilities:

• Create service plans for our clients following client assessments and admissions
• Ensure quality care is delivered to our clients by creating, implementing, monitoring and enforcing quality assurance policies and procedures
• Make necessary revisions to the plan of care as patient status and needs change
• Maintain open lines of communication between nursing shifts, schedulers, supervisors, aides, families and case managers
• Record all necessary information during each visit - client conditions, changes and status
• Ensure that arrangements for equipment and other necessary items and services are available
• Maintain client confidentiality
• Other job duties as needed

Qualifications:

• Must be 18 years of age or older
• Valid drivers license and reliable transportation
• Must be eligible to work in the U.S with an Ohio state ID
• Must have a current RN or LPN license in the state of Ohio
• Must be CPR/First aid certified
