Our Client is seeking Geriatric Case Managers to provide services in Detroit, MI and througout Wayne County.
This client is currently remote. Continuation of remote work will be re-addressed in March 2021. If remote work does not continue, 70% travel will be required with 1-2 days in the office.
SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY
* Develop and complete client's care plan; establishe goals and directives of the care plans, and services to be utilized.
* Maintain regular, ongoing contact with clients, caregivers and physicians.
* Conduct reassessments and monitor clients' progress.
* Complete and submit timely paperwork and report.
* Review and modify care plans as directed by the client/caregiver during the reassessment and obtain the clients signature of approval.
* Maintain regular, ongoing contact with clients, caregivers, physicians, and service providers listed on service plan through home visits and phone calls.
REQUIRED EXPERIENCE & EDUCATION FOR CASE MANAGER
* Licensed Social Worker (2) years of experience with prior experience in home health care, care/case management or skilled nursing home.
* Knowledge of local resources for the elderly and disabled.
* Knowledge of Medicaid and Medicare and regulations.
* Current Michigan Drivers License
