Job Description

Characteristics of Position:

All employees are expected to effectively support the agency's mission to provide consumer-centered care to those with substance disorders or with co-occurring substance and psychiatric disorders. Employees are expected to engage consumers in continuing care and treatment by using interventions that are diagnosis-specific, and communicate empathically with clients regarding addiction, mental illness, medications and recovery. Promotes client's self-determination, personal responsibility and empowerment inherent in self-directed recovery; facilitates and teaches clients role recovery elements

LOCATION: Tampa, FL

Pre-requisite Requirements:

LPN or a Bachelor's degree in a health or human services field

All ACTS, Inc. employees must be able and willing to work when expected to accommodate program and/or agency needs, participate in the agency's Quality Improvement Program and attend all assigned/required trainings, practice on-the-job safety at all times, and demonstrate the ability to communicate and work with others and be respectful of peers, supervisors, clients, guests, etc. If required by contract or administrative rule, must fully participate in NAPPI (crisis intervention) training and able to apply same to work setting.

Preferred Attributes:

Experience working in education, counseling, or wellness coaching in a medical or behavioral health setting

Physical/Safety/Health Considerations: Standard office environment and meeting rooms; work in community-based settings including client homes, primary care clinics, schools, etc.

Quality of Work Expected: Level commensurate with satisfactory Health Coach performance

Quantity of Work Expected: Commensurate with level necessary to fully meet obligations

Scope of Responsibilities: Conduct psychosocial and health assessments; provide education and information in individual or group settings to identified clients; promote consumer empowerment; link persons with needed services, benefits and entitlements, and other resources.

Supervision: Work is performed under the supervision of a Program Administrator, Director or Supervisor. This position does not provide supervision to others.

Core Responsibilities/Essential Functions:

Adheres to agency designated model for serving the health and wellness needs of persons with or at risk of substance and/or co-occurring mental disorders; promotes consumer's self-determination, personal responsibility and empowerment inherent in developing the personal knowledge, skills, and attitudes that promote health and wellness

Engages individuals in health and wellness services through motivational interviewing and supportive counseling

Screens individuals for risk of harm to self or others and provides crisis intervention when necessary

Links individuals to existing services, benefits counseling, and wellness activities to include, but not limited to, physical and behavioral health education, primary and specialty medical care, and resources to support self-management of physical and behavioral health conditions

Works as part of an interdisciplinary team to develop and implement service or treatment plans, including taking the lead to re-engage individuals who drop out of services

Provides education, case management, and supportive counseling services within areas of training, certification, education or other background in the service area; seeks direction from Program Director/Administrator when client needs fall outside those areas

Reviews and documents client response to services and adjusts interventions, level of care, and/or array of services to better meet goals, based on consultation with the individual, significant others and/or additional service providers

Modifies education and counseling to improve individual engagement and understanding, taking into consideration such characteristics as age, gender, primary language, disability, developmental level, sexual orientation or identity, ethnicity, and health status

Monitors results of health-related labs and testing and shares information with individuals and the treatment team, as applicable

Knowledge of community resources

Develops and/or maintains educational and reference materials on applicable community resources to provide to individuals receiving services

Knowledge of and compliance with specific requirements based on laws, rules, contract, and/or accreditation standards

Maintains productivity standards for assigned programs

Performs related work as required and/or assigned

Must possess valid Florida driver's license, industry accepted driving history and maintain vehicle insurance required by State of Florida

Program-Specific Scope of Responsibilities:

Conduct screening of those youths who consent to services to identify personal risk factors related to HIV, Hepatitis, and other sexually transmitted diseases, using approved screening and assessment instruments

Provide individualized education on risk reduction, identifying and emphasizing those personal and specific factors relevant to that youth

Introduce youths to the ACTS educational video on HIV and sexually transmitted diseases and provide information on how to access and view the video

Identify youths who do not have primary care services and facilitate linkage to Tampa Family Health Centers, as well as referrals for insurance and health coverage benefits

Conduct individual pre-test counseling for HIV, following the protocols of the Florida Department of Health 500/501 training

With appropriate written consent, conduct rapid HIV testing onsite at the JAC and provide post-test counseling, following the protocols of the Florida Department of Health 500/501 training

Provide follow up support, linkage, and counseling to youths who test positive for HIV and other STDs, including linkage with the Hillsborough County Health Department

Participate in health fairs and community outreach events related to HIV testing and services

Assist in data collection and reporting for research related to this program, including follow-up contacts at prescribed intervals with youths who have received services

Knowledge of confidentiality and reporting laws on results of tests for HIV, Hepatitis, and other sexually transmitted disease, and their relationship to 42 CFR Part Two and HIPAA
