JOB SUMMARY
ESSENTIAL FUNCTIONS FOR THIS POSITION
- Will work as a team with the primary care provider (PCP), medical assistant (MA) and behavioral health providers/teams
- Schedule and reschedule members for appointments with PCP or MA
- Outreach members for missed appointments and reschedule
- Fax or email referrals after PCP and MA determine need
- Monitor fax (ingoing and outgoing) and prepare forms for signature, med refill requests to medical assistant, home health forms, etc.
- Prep and review member medical records the day before to make sure all reports are available for the appointment (hospital notes, discharge summaries, etc.)
- Takes phone calls and coordinates care as appropriate
- Accurately and thoroughly documents all services and activities in the electronic medical record according to policy using approved forms and electronic systems.
EDUCATION and/or EXPERIENCE, LICENSURE and CERTIFICATION
- Bachelors degree in a field related to integrated care or equivalent combination of education, training, and/or experience of both behavioral and/or physical health.
- Proficiency in utilizing a computer including Microsoft Office programs such as Word and Outlook.
- 1-2 years previous medical experience preferred
- Experience with Microsoft Office products required
- Must have and maintain a valid Arizona driver's license
- Must be at least 21 years of age
