PRIMARY FUNCTIONS

High Needs Case Manager (HNCM) facilitates the strategic planning and Child and Family Team (CFT) process for assigned youth and families. Engages families, stakeholders, formal/ informal supports and individuals involved in the CFT on behalf of the youth and family for the primary purpose of developing a strengths-based plan which meets the youth and familys needs. As facilitator, the HNCM ensures team participation from all those involved in the clinical care of youth and family. HNCM ensures that information relevant to development and updating of the Individual Service Plan (ISP) is completed with youth, family and CFT and utilized by the entire team. Responsible for current Strengths, Needs and Culture Discovery (SNCD), ISP, crisis and safety plan and meeting frequency appropriate to the youth and family needs. Responsible for identifying any barriers to meeting a youth/familys needs and working those at the appropriate level. When an out of home (OOH) level of care is recommended by the CFT, the High Needs Case Manager is responsible for submission of the OOH packet. For children in OOH placement, the High Needs Case Manager is responsible for coordination of care with the OOH Provider and outpatient Providers and the development of discharge planning required for the return of the child to the community.

This position is established to serve as part of a team of support for youth and their families enrolled in the behavioral health system. It exists as a key role for the purpose of fulfilling The Arizona Vision: In collaboration with each child, family and others, Arizona will provide accessible behavioral health services designed to aid children to achieve success in school, live with their families, avoid delinquency and become stable and productive adults. Services will be tailored to the child and family and provided in the most appropriate setting, in a timely fashion, and in accordance with best practices, while respecting the child and familys unique cultural heritage.

MINIMUM QUALIFICATIONS

  • Knowledge of community based behavioral health services preferred.
  • Must have knowledge of spectrum of mental illnesses, DSM-IV-TR/DSM 5 codes and descriptions, psychotropic medications and crisis intervention.
  • Must maintain good consumer relationships, possess excellent verbal and written communication skills, and be able to identify priorities and stay organized
  • Meets Behavioral Health Technician criteria as defined in Office of Behavioral Health Licensing rules per the Arizona Administrative Code R9-20-204 and:
  • Has a bachelors degree and at least one year of full-time behavioral health work experience; or
  • Has an associates degree and at least two years of full time behavioral health work experience; or
  • Has over 10 years of full time behavioral health work experience.

JOB RESPONSIBILITIES AND COMPETENCIES

  • Coordinates and facilitates CFT meetings for assigned youth/families.
  • Facilitates the development of ISPs using information gained from the comprehensive assessment, the SNCD and input from CFT team members and other relevant assessments.
  • Provides information, education and support to families and team members concerning issues of creative treatment planning, coordination of care and the development and utilization of informal and community supports.
  • Ensures team meets as frequently as necessary in the youth/familys preferred natural setting to meet their needs.
  • Develops, implements and revises as necessary, a relevant crisis and safety plan for the youth/family.
  • Ensures appropriate therapist involvement on teams.
  • The High Needs Case Manager works with the youth and family to achieve independence
  • Provides information to CFT members concerning community resources and linkages to other services.
  • Assists children and families in securing needed services.
  • Maintains accurate, thorough and current documentation of contact with consumers, treatment progress and services received/provided.
  • Ensures complete and accurate progress notes of all serious incidents and changes in behavior.
  • Ensures confidentiality of verbal and written consumer information, in accordance with HIPPA standards and network policy.
  • Becomes familiar with and adheres to the legal/ethical/professional guidelines as set forth in the CPLC Policy/Procedures Manual.
  • Participates in after hours, on-call rotation.
  • Must possess current and valid fingerprint clearance card.
  • Direct supervision will be provided by clinical supervisor.
  • Must be able to work independently with discretion and judgment to conduct high needs case management for assigned families.
  • Participates in individual and group supervision.


JOB DEMANDS

Work is performed in the community. Case management services occur in families homes, schools and other community based settings as determined by youth and family. Evening or weekend hours are required on an as needed basis.
