The Hospice LVN Admissions Coordinator position is open in our Ontario location. This is a Full-time position during our day shift from 9:00am to 6:00pm.

Requirements of the Hospice LVN Admissions Coordinator:

  • Minimum one-year prior Admissions department experience (2+ preferred). Hospice experience preferred.
  • Minimum one-year prior Managed Care Insurance eligibility, verification and authorization experience.
  • Excellent customer service and communication skills.
  • Working knowledge medical terminology.
  • Ability to work efficiently and independently.
  • Must demonstrate strong organizational skills and attention to detail.
  • EMR system and Internet navigation experience; Working knowledge of Microsoft Office or Google Suite required. Data entry skills.
  • Graduate from a school of vocational nursing having State accreditation at the time of graduation and possession of a current license to practice as a vocational nurse in California required.

Hospice LVN Admissions Coordinator Duties Include:

  • Assures that all requests for Hospice services are responded to in a timely and professional manner and that all referral sources are provided with options or creative solutions for patients who present with unique or atypical needs.
  • Completes Eligibility and Insurance verification process in an accurate and timely manner.
  • Communicates with admission staff regarding initial patient needs, both verbally and in the medical record. Communicates new referrals to Hospice Team Assistants.
  • Participates in maintaining positive working relationships with contracted nursing facilities, hospitals, referring physicians, managed care organizations and private insurance companies.
  • Assists the Admissions Manager with assignment of each patient to a Nurse.
  • Participates in scheduled and ongoing meetings for improved communication and updates within the Agency.
  • Participates in the Agency Performance Improvement Program to identify areas of concern and to implement process improvements.
  • Assists Hospice Manager/Director in the maintenance of updated logs, worksheets, Agency files and calendars on a timely basis.
  • Answers incoming phone calls related to customer services and ensures consistent
  • follow-up.
