Chicanos Por La Causa provides a comprehensive senior housing and meal program, including a center-based program that offers advocacy and case management services. Along with assisting its residents through supportive services, the CPLC Senior Services program also provides its clients with a "Meals on Wheels" program and a Senior Center-Based Meal Program.

JOB SUMMARY

The coordinator is responsible for taking/scheduling new referrals, opening timely, matching the best worker with the case, managing the ongoing patient needs keeping continuity as the priority aspect of the process and coordinating care per state and federal guidelines serving as a liaison between referral sources, patients/caregivers, sales department, and agency staff. Reporting employee issues, including but not limited to: no-call no, shows, unreliability, failure to follow the schedule, and changes in days/times of visits without notification, are critical to the success of the department's standards.

QUALIFICATIONS

  • High School graduate (or equivalent).


  • A minimum of one (1) year of recent experience as a scheduling coordinator in a home care agency or another healthcare field.


  • Excellent communication skills


  • Proficiency in office applications and EMR navigation.


  • Ability multi-task and a propensity for working with and helping people


  • Must have a criminal background check.


  • Clinical background preferred.


RESPONSIBILITIES/ESSENTIAL FUNCTIONS

Answers phones, receives all intake information/enters the appropriate information on the intake/referral log and EMR system.

Takes telephone referral information and ensures that follow-up will be completed on acceptance.

Aides in redirecting referral source if unable to accept to another source/agency. Ensures Medicare eligibility, physician licensure/OIG clearance/NPI/PECOS. Verifies the status of the Primary Care Provider.

Tracks all admit and non-admits on the referral log.

Ensures recent physician orders, demographics, history, and physical are obtained and any additional information needed.

Handles all matters related to these referrals, except issues that require professional/clinical judgment: Advise the Nursing Supervisor of all new cases to be filled. Documents and reports to supervising nurse when there is a change in patient status, change in service requirement, change in employee assigned to the case, and reports any unsatisfactory performance.

Effective January 3, 2022, all CPLC Employees and all Service Providers, are required to be Fully Vaccinated against COVID-19. Newly hired employees, who are not Fully Vaccinated at the time of hire, are expected to begin a vaccination series immediately upon hire. CPLC recognizes medical and religious exemptions to the COVID-19 vaccination requirement, in accordance with applicable law and upon approval by CPLC. However, any person approved for an exemption, without exception, is subject to masking.
