POSITION SUMMARY

The Integrated Care Coordinator works collaboratively with the Behavioral Health Consultant, providers, and the Integrated Behavioral Health team. The Integrated Care Coordinator will assist in identifying patients at their respected sites who would benefit from behavioral health intervention. They also assist with gathering and providing resources, as well as other administrative needs pertaining the Integrated Behavioral Health.

Qualifications

ESSENTIAL SKILLS AND EXPERIENCE:

  • Bachelors degree in social service field is preferred or a minimum of five (5) years of behavioral health experience and one (1) year of direct experience may replace Bachelors Degree

  • High school diploma or GED from an accredited institution is required

  • Minimum of one (1) year of experience working with State AHCCCS and Regional Behavioral Health Authority (RBHA) clients; preferred

  • Excellent customer service skills

  • Excellent computer skills

  • Knowledge of Arizona Department of Health Services and RBHA requirements

  • Knowledge of chronic disease management

  • Ability to complete PRAPARE assessments

  • Comprehensive and working knowledge of community resources

  • Thorough knowledge of current versions of DSM and ICD

  • Ability to complete a Behavioral Health Assessment with appropriate clinical oversight

  • Ability to provide effective and respectful quality care and services that are responsive to diverse cultural beliefs and practices

  • Certification to perform cardiopulmonary Resuscitation for the Health Care Professional (CPR) and AED through courses that follow the guidelines from the American Heart Association and Red Cross

  • Bilingual English/Spanish strongly preferred

  • Valid Arizona drivers license

  • Valid Level One Fingerprint Clearance Card issued by Arizona Department of Public Safety

POSITION RESPONSIBILITIES

  • Participate as directed in the screening of new service requests within the center ensuring that all individuals are adequately and appropriately served according to their individual needs and in accordance with existing policies and procedures

  • Participate in regular interdisciplinary staff meetings

  • Develop appropriate treatment/service plans with clients as the means for implementing appropriate service and developing effective alliances with clients

  • Works with the medical team including the medical providers and medical assistance in the utilization of behavioral health services

  • Utilize AZARA reports to research potential patients for the Behavioral Health Consultants

  • Responds to positive screenings, triages scores, communicates plans as needed with patient and refers to BHC

  • Provide direct client services to individuals and/or family members who are in need of resources or other issues related to behavioral health needs

  • Assist in setting up transportation for patients as needed for medical or specialty behavioral health appointments

  • Assist supervisory staff with tracking of clients, including the generation of reports that will allow follow-through on quality of care

  • Document all service contacts on a timely basis

  • Assist with monitoring the assigned task boxes at location which do not have an Integrated Care Coordinator

  • Provide resources to clients contingent on their presenting needs

  • Responsible for distributing mailing to patients including after failure to contact for follow up care

  • Complete reminder calls to patients who have scheduled follow up appointment with the Behavioral Health Consultant

  • Work collaboratively with all departments as it pertains to integrated behavioral health

  • Research and disperse to the team educational information relevant to the field of integrated behavioral health

  • Perform other duties as assigned by the supervisor which are consistent with the position and in compliance with Adelante Health Care policies and procedures

Work environment: While performing the duties of this job, the employee is exposed to weather conditions prevalent at the time when traveling to various clinic sites. The noise level in the work environment is usually moderate. The employee may be subject to health hazards (contagious diseases, blood borne pathogens, etc.) when working in the clinic area.
