Internal / Adult Medicine Physician

Mountain Park Health Center is seeking Internal Medicine or Family Medicine physicians to join us as we seek to address the quadruple aim of healthcare: better outcomes, lower costs, improved patient experience and an improved clinician experience. Candidates should have a passion for community health and a desire to treat the underserved.

Brief Description

The Physician will provide high quality, comprehensive medical care to a wide variety of patients in a multidisciplinary setting. Our providers enjoy the benefits of working alongside behavioral health specialists, registered dietitians, and clinical pharmacists in a fully integrated environment.

Essential Duties & Responsibilities

Provide comprehensive, culturally competent primary care services in a patient centered medical home model.

Participate in our integrated care delivery model, helping our patients take advantage of behavioral health, nutrition and clinical pharmacy services.

Serve as a clinical leader and integral team player within the department, treating all levels of staff and patients with respect.

Provide after-hours coverage in rotation with other clinicians within the department.

Contribute to the training and proficiencies of clinical personnel, including medical students.

Adhere to all policies, procedures, and protocols, including our Code of Conduct.

Actively participate in quality improvement initiatives and contribute valuable input related to care delivery.

Attend monthly medical staff meetings.

Other duties as assigned.

Completion of an ACGME approved Residency in Family Medicine or Internal Medicine.

Board Certified/Board Eligible

Current AZ medical license

Current DEA registration

BLS Certification
