POSITION SUMMARY--Actively support and coordinate Children and Youth Mental Health Treatment Act (CYMHTA) referrals, hospital discharge intakes and triage C&F Access incoming phone calls. Participate on a multi-disciplinary child and family team that provides comprehensive services for children, adolescents and their families. The approach of the team is characterized by being strengths-based, family-centered, culturally competent and community responsive.

SALARY RANGE:

LCSW - $26.02/hr - $31.20/hr

LPC/LMFT - $25.13/hr - $30.13/hr

ESSENTIAL JOB FUNCTIONS:

  • Manage and triage C&F Access incoming phone calls.
  • Conduct face to face behavioral health assessments in the community or in the office for hospital discharge referrals to MHCD.
  • Collaborate with family, professionals and other external providers regarding hospital discharge intakes.
  • Conduct general intake assessments for Child and Family Outpatient services.
  • Provide a clinical assessment/intake for new CYMHTA referrals and follow State protocol.
  • Maintain accurate and timely clinical documentation consistent with agency standards.
  • Participate in team meetings, trainings and supervisory sessions as required.
  • Other duties as assigned

REQUIREMENTS:

EDUCATION:

Master's Degree in social work, psychology, or similar field. Current license as a LCSW/LPC/LMFT in the State of Colorado.

EXPERIENCE:

Minimum of three years experience in clinical social work. Experience working
with moderate to severe and persistently mentally ill populations.

SKILLS AND COMPETENCIES:

  • Ability to join with and implement a strengths-based approach to children, families and adults.
  • Knowledge of methods of psychotherapy.
  • Knowledge of community and available resources.
  • Ability to assess crisis situations and intervene appropriately.
  • Familiarity with the DSM V and diagnostic techniques.
  • Effective written and verbal communication skills.
  • Flexibility.
  • Self-directed individual, and team player.
  • Must have valid drivers license and good driving record.
  • Knowledge and understanding of trauma-informed care principles/practices.

SUPERVISORY RESPONSIBILITIES: Assist with program supervision of students/volunteers as determined appropriate.

TYPICAL PHYSICAL DEMANDS: Ability to sit, stand, bend and reach, drive, operate standard office equipment such as phone, computer, facsimile, copier/printer, and other business machines. Occasional lifting up to 40 lbs.

TYPICAL MENTAL DEMANDS: Ability to give, receive and analyze information, formulate work plans, prepare written materials and articulate goals and action plans. Must understand people from all walks of life and be able to communicate effectively with them.

WORKING CONDITIONS: Maintain flexible schedule. Some local travel depending on position.
