Revival Home Health Care is expanding our footprint and seeking Full-Time LPNs to join our Margate, Florida team.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Implements plan of care initiated by the registered nurse.
• Provides accurate and timely documentation consistent with the plan of care.
• Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.
• Informs the Team Lead of issues pertinent to the care plan process and any known or perceived issues.
• Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST) to the registered nurse and/or Clinical Supervisor/Nursing Supervisor.

SKILLS AND QUALIFICATIONS:

• Licensed Practical Nurse required.
• Strong interpersonal skills and ability to work as part of a team.
• Good written and verbal communication skills.
• Good computer skills including knowledge of Microsoft Office (i.e., Word, Outlook, Excel, etc.).
• Confident
• Self-Motivated
• Team Player
• Problem Solver
