Job SummaryPerforms patient observation, data collection related to patient care, administers medication, treatment and procedures, implements the plan of nursing care and communicates findings to the supervising RN. Assumes responsibility and accountability for facilitating, communicating and collaborating with the healthcare team to identify and meet the physical, emotional and spiritual needs of the patients. Promotes the optimal health, well-being and safety of the patient through use of the nursing process and in accordance with patient care standards, guidelines and the State Nurse Practice Act. Demonstrates personal accountability for relationship-based care, organizational mission, and core values.

Education Requirement
Graduate of an accredited Licensed Practical Nursing program
License/Certifications Requirements
Current state LPN license for applicable state of employment
Current Basic Life Support (BLS) certification from American Heart Association
Current state IV Certification up hire or within three month of hire/transfer
Applicants will need to consider the following facilities: Longmont United Hospital, Avista Adventist Hospital, St. Anthony's North Hospital, St. Anthony's Hospital, Porter Adventist Hospital, and Ortho Colorado.
Full time expectation is 3 shifts per week
Available 4 weekend shifts in 6 weeks
Available 5 holidays/high need shifts in a year

Medium Work - (Exert up to 50lbs force occasionally, and/or up to 20lbs frequently, and/or up to 10lbs constantly)
