Licensed Practical Nurse
Department: Case Management
Job Summary: Contributes to the assessment, planning, implementation, and evaluation of patients to deliver safe, therapeutic and age appropriate patient care. The LPN provides input to the health care team to promote wellness, maintain current health, and/or intervene in acute or chronic illness.
Minimum Education Requirements: Graduate of School of Practical Nursing.
Minimum Work Experience: At least six months of recent related healthcare experience
Equipment/Software/Certification/License: Basic computer literacy and current MA LPN license
