Performs patient observation, data collection related to patient care, administers medication, treatment and procedures according to the physicians' orders and resident care plan. Communicates patient findings to the supervisor. Assumes responsibility and accountability for facilitating, communicating and collaborating with the healthcare team to identify and meet the physical, emotional and spiritual needs of the patients. Oversees care provided by CNAs, Care Partners, QMAPs and/or other support staff. Promotes the optimal health, well-being and safety of the patient through use of the nursing process and in accordance with patient care standards, guidelines and the State Nurse Practice Act. D emonstrates personal accountability for relationship-based care, organizational mission, and core values.

One year of previous LPN experience, preferredRequires knowledge in nursing and person-centered care conceptsAbility to demonstrate competency in the provision of individual care of patients based on the patients' assessed age and developmental needs.

HS Diploma Required

Current state LPN license for applicable state of employment or current LPN license from a participating statein National Compact Licensure (Colorado only)

BLS Required

Medium Work - (Exert up to 50lbs force occasionally, and/or up to 20lbs frequently, and/or up to 10lbs constantly)

Sign On available: $10,000.
