Job Description
Summary: The Medical Records Clerk is responsible for managing patient medical charts throughout the clinic and performing all other designated medical records functions.
Responsibilities:
- Retrieves charts of established patients prior to their scheduled visits, utilizing the record locator computerized system.
- Reviews records for completeness (i.e. loose notes, missing forms, signatures, etc.).
- Responds to all telephone and/or list requests for medical records within 24 hours of the request.
- Maintain order and sequence of medical charts in designated location.
- Securing confidentiality of medical charts and patient information (upholding HIPAA standards and regulations).
- Maintain record release log as necessary ( including who requested the record, when it was requested, when the request was received, when records were sent, and other information as directed by the Health Services Manager).
- Copies and released records to requesting party in accordance with federal and state laws, maintain privacy and confidentiality.
- File loose records in charts.
- Process all form requests and maintain a special log for such requests.
- Purge old records and keep logs of such records. Inactive charts should be retained for at least seven years and records of minors must be retained for a minimum of one year after the minor had reached age 18, but in no event less than 7 years.
- Maintain effective communication and working relationships with other employees and patients.
Job Requirements:
Education: High School Diploma or GED.
Certification required: No certification required.
Experience: 1-2 years experience in a medical records office or similar
Verbal and Written Skills to perform the job: Familiarity with basic word processing, spreadsheet, and database applications. Accurate keyboarding skills.
