*MSW-LCSW, 40 Hrs/Week, Day/Evening Shifts (8:00AM to 4:30PM/12PM to 8:30PM), Weekends As Needed

Position Summary:

The Social Worker will function as a member of the interdisciplinary renal team in providing social work services to chronic renal patients in various stages of diagnosis and treatment. Social work service is responsible for the out-patient hemodialysis and peritoneal dialysis units. The social worker is a member of the interdisciplinary team caring for the out patients at the medical center when they are admitted to the inpatient units.

Licensed Clinical Social Worker Job Responsibilities:

  • Collaborates with all staff on the unit and in the medical center whenever needed for patient care, performance improvement, committees, focus groups, etc.
  • The social worker provides intake to the outpatient dialysis unit. They are responsible for tracking patients through continuum. They complete CMS documentation and collect other regulatory data. The social worker sees all patients entering the chronic phase of renal failure for initial psycho-social assessment and planning. Referrals generally are made by nephrologists.
  • Serve in a consultative role to staff on the unit and in the medical center whenever needed for patient care.
  • Provide social work services on unit(s), assisting patients and families to cope with plans and decisions that need to be made before, during, and after hospitalization or outpatient care.
  • The social worker follows each patient in and out-patient program and may assume either a case management function, provide crisis-intervention and/or ongoing-counseling services to the patient and family.
  • The social worker helps to ensure continuity of care for dialysis out-patients when they are hospitalized by provide crisis-intervention, performing liaison functions and by informing Continuing Care RN of pertinent extended care needs and on-going home care services.
  • The social worker is responsible for facilitating transportation arrangements to dialysis treatments, monitoring the appropriateness of transportation plan (i.e., continued medical necessity of ambulance) and where indicated, facilitating a change in transportation plan.
  • When treatment alternatives such as transplant, home-dialysis CAPD etc. are being considered the social worker attends family meeting and contributed to evaluation and decision making by sharing knowledge of the psycho/social by dynamics and counseling the patient as this is indicated.
  • The social worker provides in-service education to new staff regarding psycho-social implications of renal disease and provides consultation an liaison services to staff in such areas a patient management, understanding of case dynamics, eligibility for services/benefits etc.
  • The social worker plays a vital role in patient education by keeping abreast of all patient-oriented literature; coordinating and sponsoring patient education seminars with other key staff; co-leading support group for home dialysis patients.

LCSW Minimum Education and Experience:

  • Masters in social work
  • Health Care Experience

Required Certification/Licensure:

  • LCSW, LICSW
  • Licensed Clinical Social Worker must continue his/her professional education through readings, seminars, workshops, etc.

Additional Special Qualifications:

  • Must work with patients and families who are in crisis.
  • Be exposed to patients with major trauma, infectious disease, and/or emotional upset.
